摘要
目的探讨经皮肾镜超声联合钬激光气压弹道碎石治疗复杂性肾结石的临床疗效及对患者肾功能的影响。方法选择复杂性肾结石患者100例,随机分为观察组和对照组各50例。观察组行经皮肾镜超声联合钬激光气压弹道碎石治疗;对照组行开放性肾切开治疗。对比两组治疗疗效以及不同治疗方案对于患者肾功能的影响。结果观察组术中出血量及术后住院时间均显著少于对照组,结石清除率高于对照组,差异均有统计学意义(均P<0.05)。观察组患者术后并发症发生率显著低于对照组,差异均有统计学意义(均P<0.05)。两组在行不同术式进行治疗后,肾盂集合系统分离指数,肌酐以及尿素氮均有所下降,但是观察组下降程度更加明显,差异具有统计学意义(P<0.05)。结论经皮肾镜超声联合钬激光气压弹道碎石治疗复杂性肾结石疗效较好,并且能够有效缓解患者肾功能水平,安全性较高,值得临床推荐使用。
Objective To study the clinical effect of percutaneous nephrolithotomy combined with holmium laser lithotripsy for the treatment of complex renal calculi and the effect on renal function in patients. Methods Total of 100 cases of complex renal calculi patients in the hospital were selected from May in 2012-March in 2014, and which were randomly divided into observation group (50 cases) and control group (50 cases). The observation group un- derwent percutaneous nephrolithotorny combined with holmium laser lithotripsy treatment. The control group un- derwent extracorporeal shock wave lithotripsy. Results In the observation group of volume of hemorrhage and post- operative hospitalization time was significantly less than the control group, the stone free rate was higher than the control group(P〈0. 05). The observation group of patients, the incidence of postoperative complications was sig- nificantly lower than the control group(P〈0.05). The two group underwent different surgical treatment, the renal pelvis separation of the collection system index, creatinine and urea nitrogen were decreased, but the observation group decreased more significantly, and there were statistical significantly differences in two groups(P〈0.05). Conclusion Percutaneous nephrolithotomy combined with holmium laser lithotripsy for the treatment of complex re- nal calculi curative effect is better, and can effectively alleviate the level of renal function of patients, high safety, and is worthy of clinical use recommended.
