摘要
目的研究前列腺增生症患者尿路感染的病原菌分布及耐药性,为临床治疗提供依据。方法选择2013年8月-2015年8月医院收治的前列腺增生症患者250例,收集患者中段尿液,采用VIT EK-2Comtact法国生物梅里埃公司全自动微生物鉴定及药敏分析系统进行菌种鉴定,用K-B纸片法进行药敏试验。结果 250例前列腺增生症患者中,发生尿路感染106例,感染率为42.40%,共分离出病原菌120株,其中革兰阴性菌86株占71.67%,革兰阳性菌34株占28.33%,前3位病原菌分别为大肠埃希菌、屎肠球菌、肺炎克雷伯菌,分别占35.83%、15.83%、13.30%;大肠埃希菌、肺炎克雷伯菌及奇异变形菌对庆大霉素、甲氧苄啶的耐药率均>50.00%,而对亚胺培南耐药率最低,为0;粪肠球菌、屎肠球菌对左氧氟沙星、庆大霉素、环丙沙星、青霉素以及氨苄西林的耐药率均>50.00%,对万古霉素耐药率最低,为0。结论前列腺增生症患者尿路感染病原菌以大肠埃希菌最常见,该感染多数病原菌均具有较高耐药性,临床可选择万古霉素与亚胺培南联合治疗。
OBJECTIVE To study the distribution and drug resistance of pathogenic bacteria causing urinary tract infections in patients with hyperplasia of prostate,so as to provide the basis for clinical treatment.METHODS A total of 250 patients with hyperplasia of prostate in our hospital from Aug.2013 to Aug.2015 were selected,whose urine specimen were collected to have a strain identification by VIT EK-2Comtact a automotive bioassay and sensitivity test system from French Biomerieux company,and K-B was used for drug resistance test.RESULTS A total of 106 patients occurred urinary tract infection among the 250 patients with hyperplasia of prostate,and the infection rate was 42.4%.Totally 120 strains of pathogenic bacteria were detected,including 86 strains of gram negative bacteria(71.67%),and 34 strains of gram positive bacteria(28.33%).The top three pathogenic bacteria were Escherichia coli,Enterococcus faecium and Klebsiella pneumoniae,accounting for 35.83%,15.83% and 13.30%,respectively.The drug resistance rates of Escherichia coli,Klebsiella pneumoniae and Proteus mirabilis to gentamicin and trimethoprim were both〉50.00%,but with the lowest drug resistance rate to imipenem(0%).The drug resistance rates of Enterococcus faecalis and Enterococcus faecium to levofloxacin,gentamicin,ciprofloxacin,penicillin and ampicillin were all〉50.00%,but with the lowest drug resistance rate to vancomycin(0%).CONCLUSION E.coli is the main pathogenic bacteria causing urinary tract infections in patients with hyperplasia of prostate,and the pathogenic bacteria are highly resistant to many kinds of antibacterials.It is effective to combine vancomycin and imipenem for the treatment.
