摘要
目的 探析急性心肌梗死在经皮冠状动脉介入（PCI）术中冠脉内应用替罗非班的疗效。方法 选取我院2014年1月-2015年12月收治的80例急性心肌梗死患者作为研究对象,将其随机分为两组,对照组（常规PCI治疗）、试验组（PCI＋替罗非班治疗）各40例。观察两组患者临床疗效,比较冠脉血流灌注指标和心血管不良事件。结果 两组患者术后左室射血分数（LVEF）、左室舒张末内径（LVEDd）、左室收缩末内径（LVESd）比较,差异无统计学意义（P〉0.05）。试验组血流分级、TIMI心肌灌注分级、校正的TIMI血流帧数计数优于对照组;试验组不良事件发生率为5.0%（2例）,低于对照组的20.0%（8例）,差异有统计学意义（P〈0.05）。结论 PCI治疗急性心肌梗死疗效确切,术中冠脉内应用替罗非班有利于改善冠脉血流灌注,减少心血管不良事件,值得推广。
Objective To analyze the effects of tirofiban in the treatment of acute myocardial infarction in patients with percutaneous coronary intervention（PCI） therapy.Methods 80 acute myocardium infarction patients from January 2014 to December 2015 of our hospital were selected as study object.Patients were randomly divided into control group（routine treated with PCI） and experimental group（treated with PCI ＋ tirofiban）,and each group was 40 cases.The clinical effects were observed.Coronary perfusion index and adverse cardiovascular events were compared.Results The postoperative left ventricular ejection fraction（LVEF）,left ventricular end diastolic diameter（LVEDd） and left ventricular end diastolic diameter（LVESd） between two groups was compared with no statistical difference（P〈0.05）.The thrombolysis in myocardial infarction,TIMI myocardial perfusion grading and corrected TIMI flow frame number counting in experimental group was better than that of control group.The incidence of adverse events in the experimental group was 5%（2 cases）,lower than 20% in the control group（8 cases）,with statistical difference（P〈0.05）.Conclusion PCI therapy for acute myocardial infarction with definite curative effect,using tirofiban in coronary artery during operation is helpful to improve coronary perfusion,decrease adverse cardiovascular events.It is worth popularizing.
