摘要
目的:观察甲亢患者血脂水平和血清肿瘤指标水平,分析甲状腺自身免疫抗体与血脂的相关性。方法:对住院甲亢患者275例测定空腹血糖(FBG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL)、低密度脂蛋白胆固醇(LDL)和血尿酸(UA),同时检测血清甲胎蛋白(AFP)、癌胚抗原(CEA)、CA-199和CA-50,并与同期健康体检患者66例进行比较。测定甲亢血清甲状腺过氧化物酶抗体(TPOAb)、促甲状腺素受体抗体(TRAb)和甲状腺球蛋白抗体(Tg Ab)水平,分析TC和LDL与甲状腺自身免疫抗体的相关性。结果:(1)甲亢患者总胆固醇(TC)3.46±0.98mmol/L,低密度脂蛋白胆固醇(LDL)1.20±0.33 mmol/L,水平显著低于正常对照组;(2)甲亢患者血清甲状腺过氧化物酶抗体(TPOAb)和促甲状腺素受体抗体(TRAb)与血总胆固醇(TC)和低密度脂蛋白胆固醇(LDL)水平呈负相关;(3)甲亢患者血清甲胎蛋白(AFP)水平2.02±1.15μg/L,与甲状腺过氧化物酶抗体(TPOAb)及促甲状腺素受体抗体(TRAb)水平呈负相关,血清癌胚抗原(CEA)水平2.78±6.78μg/L,甲状腺球蛋白抗体(Tg Ab)水平呈正相关。结论:(1)甲亢患者的血清总胆固醇和低密度脂蛋白胆固醇水平低于正常人群;(2)甲亢患者血清甲状腺过氧化物酶抗体和促甲状腺素受体抗体与血清总胆固醇和低密度脂蛋白胆固醇水平呈负相关。(3)甲亢患者血清甲胎蛋白水平和甲状腺过氧化物酶抗体及促甲状腺素受体抗体水平负相关。
Objective：To investigate the serum levels of blood fat and tumor makers in hyperthyroidism patients and relationship between the levels of blood fat and thyroid autoimmune antibody. Methods： FBG, TG, TC, HDL, LDL, and UA were measured among 66 healthy people matched control subjects and 275 hospitalized hyperthyroidism patients, at the same time, the levels of serum AFP, CEA, CA199 and CA50 were detected also. In the hyperthyroidism patients, serum levels of TPOAb, TRAb and Tg Ab were measured simultaneously. The relationship between TC and thyroid autoimmune antibody, LDL and thyroid autoimmune antibody were analysed respectively. Results：（1） Levels of TC（3.46±0.98 mmol/L）and LDL（1.20±0.33 mmol/L） in hyperthyroidism patients were significantly lower than in the control group.（2）Serum levels of TPOAb and TRAb were negatively correlated to TC and LDL respectively in hyperthyroidism patients.（3）Serum levels of AFP（2.02±1.15μg/L） were negatively correlated to TPOAb and TRAb in hyperthyroidism patients. Serum levels of CEA（2.78±6.78μg/L） in hyperthyroidism patients were slightly correlated to Tg Ab. Conclusion：TC and LDL in hyperthyroidism patients were significantly lower than in the healthy group. The higher levels of TROAb and TRAb, the lower levels of TC and LDL were in hyperthyroidism patients. Serum levels of AFP were negatively correlated to TPOAb and TRAb in hyperthyroidism patients.
