摘要
目的探究肺保护性通气与机械通气治疗急性呼吸窘迫综合征的临床疗效。方法 选取2013年12月-2015年12月我院收治的急性呼吸窘迫综合征患者共73例,将其随机分为对照组（n=36）与观察组（n=37）,对照组给予常规机械通气治疗,观察组给予肺保护性通气治疗。比较两组患者血气指标、住院时间、通气时间、肺损伤发生率及死亡率。结果 治疗后48 h,观察组Pa O2、氧合指数（Pa O2/Fi O2）、Sp O2明显高于对照组,Pa CO2明显低于对照组（P〈0.05）。观察组住院时间、通气时间明显短于对照组（P〈0.05）。观察组呼吸机相关肺损伤发生率为5.4%,明显低于对照组的25.0%,观察组死亡率明显低于对照组（P〈0.05）。结论 肺保护性通气治疗急性呼吸窘迫综合征有较好的临床疗效,能明显改善血气相关指标,降低呼吸机相关肺损伤发生率及死亡率,值得临床推广应用。
Objective To explore the curative effect of lung protective ventilation and mechanical ventilation in the treatment of ARDS. Methods A total of 73 patients with ARDS in our hospital from December 2013 to December 2015 were randomly divided into control group（n=36） and observation group（n=37）.The control group was treated with con- ventional mechanical ventilation,the observation group were treated with lung protective ventilation.The blood gas indi- cators,length of stay,ventilation time,incidence of lung injury and mortality were compared between the two groups. Re- suits After 48 h of treatment,PaO2,oxygenation index and SpO2 in the observation group was significantly higher than that of the control group respectively,PaCO2 in the observation group was significantly lower than that in the control group （P〈0.05）.The hospitalization time,ventilation time in observation group was significantly shorter than that of the control group respectivety（P〈0.05）.The incidence of ventilator associated lung injury in the observation group was 5.4%, significantly lower than 25% of the control group,the mortality rate of the observation group was significantly lower than that of the control group（P〈0.05）. Conclusion Lung protective ventilation treatment of ARDS has better clinical effica- cy,can significantly improve the blood gas index,reduce ventilator associated lung injury incidence rate and mortality,is worthy of clinical popularization and application.
