摘要
目的:观察平喘固本汤联合双水平无创BIPAP治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭的临床效果。方法:收集来我科就诊的临床诊断为COPD合并呼吸衰竭的患者56例,采用随机数字表法将其分为治疗组与对照组,对照组仅采用双水平无创BiPAP技术治疗,而治疗组则采用平喘固本汤联合双水平无创BiPAP技术治疗。结果:治疗后两组患者的PH值和PaO_2值均有所升高,PaCO_2有所降低,且治疗后治疗组PH值和PaO_2均较对照组高,PaCO_2较对照组低,差异均具有统计学意义(P<0.05),治疗后两组患者的心率均有所降低,且治疗组的降低幅度大于对照组,差异具有统计学意义(P<0.05);治疗组的气管插管率(7.1%)低于对照组,住院时间少于对照组,差异具有统计学意义(P<0.05),治疗组的病死率低于对照组(28.6%),不良反应发生率高于对照组,但差异无统计学意义(P>0.05)。结论:平喘固本汤联合双水平无创BIPAP治疗COPD合并呼吸衰竭的临床效果显著。
Objective：To observe the clinical effect of Pingchuan Guben decoction combined with bilevel noninvasive positive pressure ventilation in the treatment of patients with chronic obstructive pulmonary disease and respiratory failure.Methods：56clinical diagnosis＇ s patients with COPD and respiratory failure were collected from our department,randomly divided into two groups,respectively the observation and control group,28 cases in the two group,the patients of the observation group were treated by Pingchuan Guben decoction combined with bilevel noninvasive positive pressure ventilation,while the control group patients were treated with bilevel noninvasive positive pressure ventilation alone.Results：The pH and PaO_2 value in the two groups of patients after treatment were increased,PaCO_2 decreased,and after treatment,pH and PaO_2 in the observation group were higher than the control group,PaCO_2 was lower than the control group,the difference was statistically significant（P0.05）,after treatment,the heart rate of the two groups were decreased,and the observation group was lower than the control group,the difference was statistically significant（P0.05）,tracheal intubation rate（7.1%）in the observation group was lower than the control group（28.6%）,the hospital stay was less than the control group,the difference was statistically significant（P0.05）,the mortality rate in the observation group was lower than the control group,the incidence of adverse reactions was higher than the control group,but the difference was not statistically significant（P0.05）.Conclusion：The clinical effect of Pingchuan Guben decoction combined with bilevel noninvasive positive pressure ventilation in the treatment of patients with chronic obstructive pulmonary disease and respiratory failure is significant.
