心肌酶检测评价急性呼吸道感染并心肌损害 被引量:18

Assessment of Acute Upper Respiratory Infection Accompanied With Myocardial Damage by Detection of Myocardial Enzymes

摘要 目的 观察ARI并心肌损害患儿心肌酶及其同工酶和ECG的动态改变 ,以探讨早日确诊ARI并心肌损害的敏感指标。方法 对 117例ARI疑似或确诊心肌损害的损害的患儿和 6 0例健康同龄儿童动态监测心肌酶及其同工酶谱和ECG。结果 两组心肌酶及其同工酶定量测定结果比较 :CK -MB、LDH - 1、AST明显升高 ,(P <0 0 0 1)。两组心肌酶及其同工酶异常率比较 :ARI组CK -MB、LDH - 1、AST异常率高于对照组 ,(P <0 0 1或P <0 0 0 1)。两组ECG异常率比较 :ARI组ECG异常率高于对照组 ,(P <0 0 1)。ARI组ECG改变与心肌酶及其同工酶改变的比较 :心电图严重异常者 ,CK -MB、LDH - 1、AST增高明显 ,P <0 0 1,差异有统计学意义。ARI组中ECG异常与正常的心肌酶及其同工酶异常率比较 :CK -MB、LDH - 1和AST的异常率在ECG异常者与ECG正常者中比较。P均 <0 0 1。结论 心肌酶CK -MB、LDH - 1、AST是判断ARI并心肌损害的重要依据 ,CK-MB、是确定心肌损害的关键指标 ;心肌酶及其同工酶增高的幅度及ECG改变的严重程度与心肌损害有关联 ;ECG改变的严重程度及CK -MB、LDH - 1、AST的增高幅度可作为心肌损害的敏感指标 ,也是判断预后的重要手段。 Objective To investigate the dynamic changes of myocardial enzymes and it's isoenzyme,eletrocardiogram(ECG) for suffering children of ARI accompanied with myocardial damage and to determine the sensitive indexes of confirmed diagnosis of ARI accompanied with myocardial damage.Methods 117 cases with ARI accompanied with suspected or confirmed myocardial damage were studied,60 healthy age matching children were arranged as control group.Myocardial enzymes and it's isoenzyme ECG in the two groups were dynamic monitored.Then comparing the activities of serum myocardial enzymes and it's isoenzyme.Results Serum CK-MB,LDH-1,AST were significantly higher in ARI group than that in control group(P<0 001).The abnormal ratioes of CK-MB,LDH-1,AST were significantly different.In ARI group,the values of CK-MB,LDH-1,AST in patients with serious ECG abnormality were more higher,the diffenence was significant(P<0 01).Conclusions Myocardial enzymes of CK-MB,LDH-1,AST are the important evidence of myocardial damage.CK-MB is a key index to diagnose myocardial damage.The increament range of serum myocardial enzyme and it's isoenzyme activities are positive correlated with the degree of myocardial damage.The ECG changes and the increasing degree of CK-MB,LDH-1,AST are the sensitive indexes as well as the important means to judge prognosis of myocardial damage.
