病毒性脑炎患儿血清酶学变化的临床意义 被引量:2

摘要 目的:通过对小儿病毒性脑炎心肌酶谱的检测,探讨血清心肌酶含量与病毒性脑炎患儿病情轻重程度的相关性及其临床意义。方法:自2 0 0 1年1月~2 0 0 4年1月将临床诊断为病毒性脑炎的95例患儿,根据病情分为轻度4 8例、中度30例、重度17例3组,并选择健康体检儿童5 0例为对照组。使用酶动力法对以上4组进行血清天冬氨酸氨基转移酶(AST)、α-羟丁酸脱氢酶(α-HBDH)、乳酸脱氢酶(LDH)、肌酸磷酸激酶(CK)、肌酸激酶同工酶(CK -MB)检测。就其心肌酶谱均值以MATLAB软件进行统计学处理。结果:95例病毒性脑炎患儿中有5 8例(约6 .1% )心肌酶1项以上增高。对各组心肌酶谱中各酶均值进行总体方差分析,结果为各酶的活性变化在4组均有极显著性差异(P <0 . 0 0 1)。各组间心肌酶均值多重比较显示,CK在各组间均有显著性差异(P <0 . 0 5 ) ;各酶与脑炎相关性较强,尤其是α-HBDH、LDH相关系数最大,接近1。结论:心肌酶谱中各酶与病毒性脑炎密切相关,其含量随病情加重而增加。说明心肌酶学不只是心肌损伤的指标,对小儿病毒性脑炎判断病情严重程度有重要的意义。 Objective: To determine the cardiac enzymes in children with virus encephalitis,and explore the relationship between the content of serum cardiac enzymes and the seriousness of virus encephalitis and its clinical significance.Method:From January 2001 to January 2004,95 cases clinically diagnosed as children virus encephalitis were divided into 3 groups based on the states:mild group (n=48),moderate (n=30) and serious group (n=17).50 healthy children were enrolled as control group.The serum AST,α-HBDH,LDH,CK and CK-MB in 4 groups were determined by enzyme dynamics methods.The average cardiac enzymes were analyzed with MATLAB software.Results:58 of 95 cases (61%) presented elevating at least one parameter of cardiac enzymes.A general mean square analysis performed in every average enzyme showed markedly statistical differences among 4 groups (P<0.001).The multiple comparison showed significant differences between CK group and other ones (P<0.05).Every enzyme was closely reld to encephalitis,espcially α-HBDH and LDH with relative index of about 1.Conclusion:Every cardiac ate enzymes is closely related to children virus encephalitis and its content increases by the seriousness of disease.The study indicates that cardiac enzymology is a predictor for cardia clesion.It is also valuable for judging the seriousness of virus encephalitis in children.
