摘要 目的分析动脉波形连续心输出量(PICCO)监测在ICU严重心力衰竭患者中的应用效果和护理。方法选择2014年1月至2015年3月我院ICU收治的严重心力衰竭患者37例作为研究对象,均行PICCO监测,观察患者监测前后PICCO监测指标变化情况,并比较不同预后患者PICCO监测指标的差异。结果 37例严重心力衰竭患者中死亡15例,死亡率为40.54%。PICCO监测撤离后,CI、CFI显著高于监测时,ITBVI、EVLEI显著低于监测时(P<0.05)。存活组和死亡组CI、CFI比较差异无统计学意义(P>0.05),但存活组ITBVI、EVLEI显著低于死亡组(P<0.05)。结论 PICCO监测可反映出严重心力衰竭患者心功能及预后情况,应做好PICCO监测的护理,以提高监测效果。 Objective To analyze the application effect and nursing of arterial pulse-indicated continuous cardiac output (PICCO) monitoring on patients with severe heart failure in ICU. Methods From January 2014 to March 2015, thirty-seven patients with severe heart failure treated in ICU of our hospital were selected. All patients were implemented PICCO, and the changes of PICCO indices before and after monitoring were observed, the difference of PICCO indices in different prognosis patients was compared. Results Of 37 patients with severe heart failure, 15 patients died, the mortality was 40.54%. After PICCO monitoring, the CFI and CI significantly increased, the ITBVI and EVLEI significantly decreased, P〈0.05. The CFI and CI of survival group and death group had no statistical difference (P〉0.05), but the ITBVI and EVLEI of survival group were significantly lower than those of death group (P 〈0.05). Conclusions PICCO monitoring can reflect the heart function and prognosis of patients with severe heart failure. The nursing of PICCO monitoring should be done well, in order to improve the monitoring effect.
