临床路径管理对糖尿病患者住院天数和住院费用的影响 被引量:4

Impact of Clinical Pathways Management on Hospitalization Days and Expenses

摘要 目的:探讨临床路径管理对糖尿病患者住院天数和住院费用的影响,为临床治疗提供依据。方法:选择2012年1月~2014年12月本院收治的2型糖尿病住院患者245例,按照随机数字表法分为观察组和对照组,观察组实施临床路径管理,对照组按照一般治疗方法管理。比较两组患者住院天数、住院费用,血糖监测技术、胰岛素注射技术掌握情况以及患者满意度。结果:观察组患者住院天数和住院费用均少于对照组,差异有统计学意义(t=-15.700、-19.688;P〈0.05)。观察组患者血糖监测技术和胰岛素注射技术掌握率均为100%,显著高于对照组93.91%和86.96%,差异有统计学意义(Z=-13.450,-12.152;P〈0.05)。观察组患者总满意度为100%,高于对照组总满意度98.26%,差异有统计学意义(Z=-14.098;P〈0.05)。结论:对糖尿病患者实施临床路径管理,能够减少患者住院天数、降低住院费用、提高患者自我管理能力、提高患者满意度,值得临床推广借鉴。 Objective: To explore the impact of clinical pathways management on the hospitalization days and expenses, and provide evidence for clinical therapy. Methods: A toal of 245 patients with type 2 diabetes, who wee treated in Handan Sixth Hospital from January 2012 to December 2014, were selected and randomly divided into observation group and control group. The observation group was managed by clinical pathways, while the control group, by general treatment method. The hospitalization days, hospitalization expenses, master rate of blood glucose monitoring and insulin injection and the satisfaction were compared between the two groups. Results:The hospitalization days and hospitalization expenses of observation group were less than those of control group, and the differences were statistically significant(t=-15.700,-19.688; P〈0.05). The master rates of blood glucose monitoring and insulin injection in the observation group were both 100%, which were higher than that(93.91%) and that(86.96%) in the control group, the differences were statistically significant(Z=-13.450,-12.152; P〈0.05). Total satisfied rate in the observation group was 100%, which was higher than that(98.26%) in the control group, the difference was statistically significant(Z=-14.098; P〈0.05). Conclusions: Implementing clinical pathway management can reduce the hospitalization days, cut down the hospitalization expenses, improve self-management ability and the satisfaction in patients with diabetes, which is worthy of reference for clinical promotion.
