摘要 目的探讨白细胞介素23A(interleukin-23A,IL-23A)和IL-17A在不同等级子宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)及子宫颈鳞状细胞癌中的表达水平及相关性。方法收集临床标本89例,其中20例健康对照(无感染或病变),11例低级别CIN,36例高级别CIN(CINⅡ-Ⅲ);22例子宫颈鳞状细胞癌。ELISA检测血清及子宫颈分泌物中IL-23A和IL-17A的表达及其相关性。第二代杂交捕获试验检测子宫颈分泌物中人乳头状瘤病毒(human papillomavirus,HPV)DNA的表达情况,分析IL-23A和IL-17A的表达与临床预后及HPV感染的关系。结果与对照组比较,IL-23A和IL-17A在CIN和子宫颈癌分泌物及血清中呈高表达,差异有统计学意义(均P〈0.05)。子宫颈癌患者血清IL-23A与IL-17A的表达呈正相关(r=0.865,P〈0.01),子宫颈癌患者血清IL-23A、IL-17A高表达患者的生存时间明显缩短(均P〈0.01)。IL-23A和IL-17A的表达与HPV的感染有关(P〈0.01)。结论 IL-23A和IL-17A在子宫颈上皮内瘤变和子宫颈癌中高表达,有望作为早期预测子宫颈病变的生物标志物。 Objective To investigate the expressions of IL-23 A and IL-17 A in cervical intraepithelial neoplasia(CIN) and squamous cervical carcinoma(SCC) and their clinical significance. Methods Twenty-two patients with squamous cervical carcinoma( SCC),47 patients with cervical intraepithelial neoplasia( including 11 with CIN I and 36 with CIN Ⅱ-Ⅲ) and 20 healthy women( control group) were enrolled in this study. The IL-23 A and IL-17 A levels in the cervical secretions and serum were examined by enzyme-linked immunosorbent assay( ELISA). Human papillomavirus( HPV)DNA in cervical smears was detected by PCR screening kits. The correlations of IL-23 A and IL-17 A levels with cervical disorders and with HPV infection were analyzed. Results Compared with the control group,IL-23 A and IL-17 A levels were both increased in cervical secretions and in serum of patients with CIN and SCC( P〈0. 05). The levels of IL-23 A and IL-17 A in the serum of SCC patients were positively correlated( r = 0. 865,P〈0. 01). The IL-23 A and IL-17 A levels were negatively associated with the survival of SCC patients( P〈0. 01). IL-23 A and IL-17 A levels were associated with HPV infections( P〈0. 01). Conclusion Elevated levels of IL-23 A and IL-17 A in cervical secretion and serum are observed in CIN and SCC patients,which may serve as additional biomarkers for diagnosis and prognosis of cervical disorders.
