摘要
目的:探讨身痛逐瘀汤加减方预防老年粗隆间骨折围手术期下肢深静脉血栓形成的临床疗效。方法:将120例患者随机分为试验组及对照组,各60例。试验组自入院起除手术当天外,均服用身痛逐瘀汤加减方,对照组除手术当天外均皮下注射低分子肝素钙4000U,观察比较2组之间下肢深静脉血栓的形成率、VSA评分、D-二聚体及凝血酶时间。结果:试验组术后下肢深静脉血栓形成发生人数为5例,发生率8.3%,对照组发生4例,发生率为6.7%,P>0.05;术后2组VSA评分对比说明术后2d及3d后,服用身痛逐瘀汤加减方的患者疼痛较对照组疼痛减轻;术前及术后第7d D-二聚体及凝血酶时间(TT)对比P>0.05,说明身痛逐瘀汤加减与低分子肝素组抗凝效果相当。结论:围手术期老年粗隆间骨折术后患者使用身痛逐瘀汤加减可有效预防下肢深静脉血栓形成,降低患者术后疼痛、抗凝效果与4000U低分子肝素钙皮下注射相当。
Objective To discuss the clinical effectiveness of Shentong Zhuyu decoction in preventing DVT af- ter replacement of intertrochanteric fractures in the elderly patients. Methods： 120 patients were divided in- to study group（60 cases）and regular treatment group （60 cases） by simple randomization, except for the day of surgery Since admission ,the test group were taking Shentong Zhuyu decoction, the control group Subcutaneous injection of calcium low molecular weight heparin about 4000U, VSA score, D- dimer and thrombin time,formation rates observed between the two groups of deep venous thrombosis. Results： The test group, the number of postoperative lower limb deep vein thrombosis occurred for five people, the incidence was 8.3%, the control number for 4 people, the incidence was 6.7%, P〉0.05; Study on two groups of＇VSA score, Pain degree of test group cases were relieved compared with control group at the secondand third day after surgery, contrasting in the D- dimer and thrombin time （TT） of preoperative and postop- erative seventh ,there were not significant difference, P〉 0.05, Instructions in terms of anticoagulant effect, the observ- ing group and control group therapeutic equivalence. Conclusion： In the time period surrounding the elderly postopera-tive patient of intertrochanteric fracture using Shentong Zhuyu decoction can effectively prevent deep vein thrombosis of lower limb,reduce the patients＇ postoperative pain Its anticoagulant effect equivalent the effect about 4000 u subcu- taneous injection of low molecular heparin calcium.
