摘要
目的:探讨加味补阳还五汤内服治疗股骨粗隆间骨折术后隐形失血的临床疗效。方法:选择2017年1月—2018年12月接受股骨近端防旋髓内钉(PFNA)内固定术治疗的120例股骨粗隆间骨折患者,采用随机数字表法分为观察组和对照组,每组60例。对照组术后给予常规治疗,观察组在对照组基础上给予加味补阳还五汤内服治疗。比较两组患者术前、术后2周血红蛋白(Hb)和红细胞压积(HCT)水平;计算两组患者的隐性失血量。结果:两组患者术前Hb,HCT水平比较差异无统计学意义(P>0.05)。两组患者术后2周Hb,HCT水平均明显低于术前(P<0.05),但观察组Hb,HCT水平明显高于对照组,差异均有统计学意义(P<0.05)。观察组患者的隐性失血量明显少于对照组,差异有统计学意义(P<0.05)。结论:加味补阳还五汤内服能够促进患者血红蛋白的恢复,有效改善患者隐性失血情况。
Objective:To investigate the clinical efficacy of Jiawei Buyang Huanwu Decoction in the treatment of invisible blood loss after intertrochanteric fracture.Methods:All 120 patients with femoral intertrochanteric fractures admitted to hospital were enrolled in the study.All patients underwent femoral proximal anti-rotation intramedullary nail(PFNA)internal fixation in our hospital from January 2017 to December 2018.The patients were divided into observation group and control group by random number table method,60 cases in each group.The patients in the control group received routine treatment after op eration,and the patients in the observation group were treated with Jiawei Buyang Huanwu Decoction on the basis of the treatment of the control group.Hemoglobin(Hb)and hematocrit(HCT)levels were compared between the two groups before and 2 weeks after surgery.The amount of recessive blood loss was calculated in the two groups.Results:There was no significant difference in Hb and HCT levels between the two groups of patients before surgery(P>0.05).The Hb and HCT levels in the two groups after surgery were significantly lower than before surgery(P<0.05),and the Hb and HCT levels in the observa-tion group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The amount of hidden blood loss in the observation group was significantly less than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Jiawei Buyang Huanwu Decoction can promote the recovery of hemoglobin in patients and effectively improve the hidden blood loss of patients.
