摘要
目的评估地佐辛联合丙泊酚和依托咪酯作为胃镜检查麻醉策略的优势及可行性。方法将120例行胃镜检查患者随机分为3组,地佐辛＋丙泊酚组（I组,40例）,地佐辛＋依托咪酯组（Ⅱ组,40例）,地佐辛＋丙泊酚＋依托咪酯组（Ⅲ组,40例）。分别记录各组诱导时间和苏醒时间,并记录各组给药前、检查中、检查后（1～2 min）平均动脉压（MAP）、心率（HR）、脉氧饱和度（SPO2）,观察并记录各组不良反应发生。结果各组诱导时间及苏醒时间差异无统计意义（P〉0.05）;在血流动力学方面,与Ⅱ、Ⅲ组比较,Ⅰ组术中、术后MAP、HR较低（P〈0.05）;静脉注射痛、呼吸抑制发生率较Ⅱ、Ⅲ组高（P〈0.05）,II组术中体动和肌颤、恶心呕吐发生率高于Ⅰ组和Ⅲ组,Ⅲ组体动、恶心呕吐、肌颤发生率最低,循环稳定。结论地佐辛联合丙泊酚和依托咪酯更适合无痛胃镜检查,既可减少不良反应发生率,又可保持稳定的血流动力学。
Objective To evaluate the advantage and posibility of using dezocine combined with propofol and etomidate as the anesthesia for gastroscopy. Methods 120 patients who undergo the gastroscopy were divided into 3 groups randomly, which were dezocine ＋ propofol group（GroupⅠ, 40 patients）, dezocine ＋ etomidate group（Group Ⅱ, 40 patients）, and dezocine ＋ propofol ＋ etomidate（Group Ⅲ, 40 patients） respectively. The induction timescale and waking timescale of each group were recorded sepearately, and Mean Arterial Pressure（MAP）, Heart Rate（HR） and Pulse Oxygen saturation（SPO2） of each group were recorded before medicine we mentioned were given, as well as the adverse reactions occurred of each group during the examination and after the examination were observed and recorded. Results No significant difference of induction timescale and waking timescale were observed between each group（P〈0.05）. In terms of hemodynamics, the MAP and HR of Group I are lower than Group Ⅱ and Ⅲ both during and after the operation（P〈0.05）, and the pain for intravenous injection as well as the occurrence rate for respiratory depression was higher than Group Ⅱ and Ⅲ（P〈0.05）. Comparing with Group Ⅰ and Ⅲ, the occurrence rate of body moving, muscle tremor as well as nausea and vomiting in Group Ⅱ were higher than Group I and Ⅲ（P〈0.05）. The body moving, muscle tremoras well as nausea and vomiting in Group Ⅲ was the lowest, and Cycle was stable. Conclusion Dezocine combined with propofol and etomidate as the anesthesia solution is more suitable for painless gastroscopy, it can reduce the incidence of adverse reactions and keep the stability of hemodynamics.
