摘要
目的通过术前应用地佐辛观察趾部切口疼痛模型大鼠的镇痛效果和血液中NO及P物质浓度的变化,探讨地佐辛超前镇痛的作用机制。方法健康成年雄性Wistar大鼠24只,体重250~300g,随机分为3组(每组8只):0.9%氯化钠溶液组(A组),切口疼痛模型前30min腹腔注射0.9%氯化钠溶液2ml;地佐辛术前组(B组),切口疼痛模型前30min腹腔注射地佐辛0.1mg/kg2ml;地佐辛术后组(C组),切口疼痛模型后30min腹腔注射地佐辛0.1mg/kg2ml;3组均按Brennan法制作切口疼痛模型。术后30min观察动物疼痛行为改变,以累积疼痛评分评定疼痛行为。术后1h抽取大鼠尾部静脉血2ml,测定其中NO及P物质浓度的变化。结果累积疼痛评分和血液标本中NO及P物质B、C组与A组比较,差异有统计学意义(P<0.01),B组和C组比较,差异有统计学意义(P<0.05)。结论大鼠趾部切口疼痛模型术前和术后30min腹腔注射地佐辛均有明显的镇痛作用,镇痛效果与降低血液中NO及P物质的浓度相关,二者比较,手术前30min给药即超前镇痛作用更显著。
Objective To investigate the action mechanism of preemptive analgesia of dezocine by observing the analgesia effect of dezocine on rats model with plantar incision before operation and observing the changes of nitric oxide (NO) and substance P in blood. Methods Twenty-four healthy male adult Wistar rats (250-300g) were randomly divided into three groups,with 8 rats in each group. In normal saline group (group A), 0.9% normal saline 2mt was intraperitoneally injected into the rats, 30rain before the incision; in dezocine preoperative application group ( group B), dezocine 2ml (0.1 mg/kg) was injected intraperitoneally, 30min before the incision;in dezocine postoperative group (group C), dezocine 2ml (0.1 mg/kg) was injected intraperitoneally, 30min after the incision,The animal models with incision pain were established by means of Brennan method for the three groups. The changes of behaviors of the rats were observed 30min after operation and evaluated by means of cumulative pain score,and the tail venous blood 2ml of the rat was taken out 1 b after the operation, then the levels of NO and substance P in blood were detected. Results As compared with those in group A, the levels of NO and substance P in blood in group B and group C were obviously decreased, especially in group B ( P 〈 0.05 ). There were significant differences between group A and group B, C ( P 〈 0.01 ), and there were significant differences between group B and group C ( P 〈 0.05 ). Conclusion Injecting with dezocine intraperitoneally 30min before or after incision has obvious analgesic effect,which is related with the decrease of concentration of NO and substance P in blood, and the analgesic effect of preoperative administration 30min before operation (preemptive analgesia ) is more obvious.
