摘要
目的探讨湿润伤口灵联合负压封闭引流(VSD)治疗难愈性伤口的临床效果。方法选取孝感市中心医院骨科自2011年1月至2013年12月收治的85例皮肤缺损患者,按随机数字表分法分为观察组(n=43)和对照组(n=42)。对照组按常规方法换药治疗;观察组经清创后予VSD处理,1周后根据情况拆除并使用湿润伤口灵中药外敷治疗。比较并分析两组愈合率、愈合时间、愈合质量(包括皮肤愈合的颜色,硬度及张力)、伤口并发症、疼痛评分(VAS评分)等。结果观察组的愈合率为95.3%,明显高于对照组的69.0%,两组间比较,差异有统计学意义(P<0.05);两组愈合时间比较,观察组明显优于对照组,差异有统计学意义(P<0.05);两组愈合后的皮肤质量比较,观察组优于对照组,差异有统计学意义(P<0.05);两组并发症及疼痛评分比较,观察组明显优于对照组,差异有统计学意义(P<0.05)。结论湿润伤口灵联合VSD治疗难愈性皮肤缺损,可明显缩短愈合时间,改善愈合质量,减少经济负担,是一种有效的治疗皮肤缺损的方法。
Objective To explore the clinical effect of Shirunshangkouling combined with the vacuum sealing drainage（ VSD） on the treatment of the hard healing wounds.Methods All 85 patients whose skin were defect from January 2011 to December 2013,and they were divided into two groups randomly.Cases in the control group（ Group A） were treated by the traditional way.Cases in the observation group（ Group B） were treated by VSD after debridement,and VSD was removed at one week after operation.Meanwhile,we used the Chinese herbal to cover the surface of the wounds. Healing time,healing quality which included the color,hardness and tension,complication and VAS scale were compared between the two groups.Results Wound healing percent in Group B（ 95. 3%） was significantly higher than that of Group A（ 69. 0%）,as well as the healing time,and the skin quality of Group B was better than that of Group A（ P〈0. 05）.Complication and VAS scale in Group A were significantly lower than that in Group B（ P〈0. 05）.Conclusion To cure the hard-healing wounds,combined application of Shirunshangkouling and VSD is an effective approach for the treatment of hard healing wound.It can get good curative effect,shorten the healing time,improve the healing quality,and reduce the economic burden.
