慢性心衰辨证与NYHA分级及NT-proBNP相关性研究 被引量:12

摘要 目的探讨慢性心力衰竭以心气虚,气阴两虚和心阳虚为主的患者NYHA(心功能)分级及血浆N末端B型利钠肽原(NT-Pro BNP)与中医分型之间的关系。方法将符合Framingham的心衰诊断标准患者进行NYHA分级,同时用化学发光法测定血浆NT-pro BNP水平。比较不同证型心功能分级与NT-pro BNP水平的差别,检验不同证型与心功能分级及NT-pro BNP的相关性。结果 90例心力衰竭患者中,气阴两虚为主患者心功能分级低于气虚证为主患者,心阳虚为主患者心功能分级低于气阴两虚为主患者,心功能分级与证型相关性明显,心气虚为主患者NT-pro-BNP水平明显低于气阴两虚为主患者,气阴两虚为主患者NT-pro BNP水平明显低于心阳虚证患者。结论慢性心衰气虚证、气阴两虚证及阳虚证与心功能分级及NT-Pro BNP密切相关,阳虚证患者病情重于气阴两虚证患者,气阴两虚证患者病情重于气虚证患者。 Objective To investigate the relational between NT-proBNP level, classifiction of NYHA and deficiency of vital energy,deficiency of both vital energy and yin and yang deficiency in chronic heart failure. Methods Patients diagnosed chronic heart failure by Framinham criterion were classified by grades of NYHA. Their NT-proBNP levels in plasma were tested by chemical luminescence method.The heart function and the NT-proBNP level were compared between different TCM types of syndrome. Results It was lowest in patients with deficiency of vital energy that classification of the heart function and the NT-proBNP level in plasma. It was relatively lower in patients with deficiency of vital energy than patients with deficiency of both vital energy and yin that classification of the heart function and the NT-proBNP level in plasma. Conclusion It was obvious in relativity between classification of NYHA, NT-proBNP levels and TCM types of syndrome. The symptoms of patients with yang deficiency were the most severe in TCM types of syndrome. The symptoms of patients with deficiency of both vital energy and yin were the more severe than those of deficiency of vital energy.
