摘要
目的探讨血清Ⅰ型前胶原(procollagen typeⅠ,PCⅠ)含量检测在早期诊断前列腺癌骨转移中的临床价值。方法采用羊Ⅰ型前胶原放射免疫试剂联合Ⅰ型前胶原氨基端肽(procollagen typeⅠamino terminal peptide,PⅠNP)和Ⅰ型前胶原羧基端肽(procollagen typeⅠcarboxyl terminal peptide,PⅠCP)对262例样本(包括50例正常人、62例前列腺增生症、67例前列腺癌以及83例前列腺骨转移患者的血清)进行含量检测并统计分析。结果前列腺癌骨转移组PCⅠ、PⅠNP、PⅠCP含量均显著高于其余各组(P<0.001);前列腺癌组PCⅠ、PⅠNP含量均显著高于正常对照组和前列腺增生组(P<0.05),PⅠCP含量无统计学差异(P>0.05);前列腺增生组的PCⅠ、PⅠNP、PⅠCP含量与正常对照组比较无统计学差异(P>0.05);前列腺癌骨转移组PCⅠ、PⅠNP、PⅠCP含量升高的比例均显著高于前列腺增生组和前列腺癌组(P<0.01)。PCⅠ、PⅠNP、PⅠCP含量检测对前列腺癌骨转移的诊断灵敏度分别为90.4%、88.0%、84.3%,特异度分别为94.4%、96.1%、95.5%,Youden指数分别为84.8%、84.0%、79.9%,诊断正确率均在92%以上;PCⅠ与PⅠNP、PⅠCP间的灵敏度、特异度等指标比较均无统计学差异(P>0.05)。结论自主研制的羊PCⅠ放射免疫试剂有望替代进口PⅠNP、PⅠCP放射免疫试剂进行血清PCⅠ含量检测,应用于前列腺癌骨转移的早期诊断和病程监测。
Objective To study the expression and clinical value of serum procollagen type Ⅰ( PCⅠ) in the early diagnosis of bone metastases in prostate cancer patients. Methods Serum PC Ⅰ,procollagen type Ⅰ amino terminal peptide( P Ⅰ NP) and procollagen type Ⅰ carboxyl terminal peptide( PⅠCP) in 262 samples( 50 healthy subjects,62 patients with benign prostatic hyperplasia,67 patients with prostate cancer,and 83 prostate cancer patients with bone metastases) were detected by radioimmunoassay kits( PC Ⅰ kits were prepared in our laboratory,and P Ⅰ NP and P Ⅰ CP kits were purchased from Orion Diagnostics),and statistical analysis was performed. Results The PCⅠ,PⅠNP and PⅠCP levels in the prostate cancer bone metastases group were significantly higher than those in the other groups( P〈0. 001).The PCⅠ and PⅠNP levels in the prostate cancer group were significantly higher than those in the normal control group and the benign prostatic hyperplasia group( P〈0. 05),but the PⅠCP level had no significant difference among the groups( P〈0. 05). The PCⅠ,PⅠNP and PⅠCP levels had no significant difference between the benign prostatic hyperplasia group and the normal control group( P〈0. 05). The elevated percentages of PCⅠ,PⅠNP and PⅠCP levels in the prostate cancer bone metastases group were significantly higher than those in the benign prostatic hyperplasia group and the prostate cancer group( P〈0. 01). The sensitivity of PCⅠ,PⅠNP and PⅠCP levels in diagnosis of prostate cancer bone metastases was 90. 4%,88. 0%,and 84. 3%,respectively,the specificity was 94. 4%,96. 1%,and 95. 5%,respectively,and the Youden indexes were 84. 8%,84. 0%,and 79. 9%,respectively. The diagnostic accuracy was above 92%.There were no significant difterences in the sensitivity and specificity among PCⅠ,PⅠNP and PⅠCP( P〈0. 05). Conclusion The PCⅠ kits prepared in our laboratory can replace imported PⅠNP and PⅠCP kits to detect the PCⅠ levels,and PCⅠ may be a significant marker in the early diagnosis and monitoring of bone metastases in prostate cancer patients.
