摘要
本文报道17例急性非淋巴细胞白血病(ANLL)患者用小剂量阿糖胞苷(LDAra-c)治疗后,5例完全缓解,4例部分缓解。LDAra-C化疗期间10例患者骨髓抑制,出血加重。16例发生感染。14例ANLL离体白血病细胞加低浓度阿糖胞苷悬浮培养,未发现确切的诱导分化作用。但结合临床表现和实验结果,作者认为LDAra-C治疗ANLL的机理以细胞毒作用为主,可能有诱导分化作用参与。
Low dose Ara-C(LDAra-C,Ara-C 10-15mg/12h, i.m. ×14-21 days)was used in thetreatment of 17 patients with ANLL, andfrom 14 patients blast cells were isolatedbefore chemotherapy and cultured in thepresence of Ara-C(10^(-8) and 10^(-7) mol/L),nodefinite inductionof differentiation of leu-kemic cells was found. Of the 17 cases, 5obtained CR, only one of them with M_4 en-tered CR without pancytopenia or bone mar-row apdasia during the LDAra-C treatment.For the rest, 4 with M_2 obtained PR, and 8did not respond. Toxic effects, including pan-cytopenia and bone marrow aplasia, wereobserved in most of the patients. Laboratoryresults showed that differentiation was notinduced by Ara-C in low concentration inin vitro culture. LDAra-C is thought to beeffective in the treatment of ANLL, and themechanism may involve both the cytotoxici-ty and the induction of differentiation ofleukemic cells, but the former is far moreimportant.
