摘要
目的:评价四动五步法治疗单纯尺骨干闭合性骨折的临床疗效及应用价值。方法:选取西安交通大学医学院附属红会医院中医骨科及创伤上肢病区、陕西省荣誉军人康复医院外科、陕西中医药大学第一附属医院骨伤科,自2010年3月1日至2014年6月3日的176例早期单纯尺骨干闭合性骨折病例,采用多中心观察性分析临床研究方法,患者按治疗方法分为两组:四动五步组98例,采取四动五步法系列疗法进行治疗。手术组78例,采用切开复位内固定术治疗。治疗3个月后参照成人前臂骨折治疗效果Anderson评定标准进行疗效评价,并比较两组的骨折愈合时间、并发症发病率等情况。结果:四动五步组和手术组临床总优良率分别为97.75%和96.55%,两组总优良率比较差异无统计学意义(P>0.05)。四动五步组患者的骨折愈合时间明显短于手术组,两组比较差异有统计学意义(P<0.05)。结论:四动五步法治疗尺骨干闭合性骨折疗效显著,具有操作简便、费用低廉、疗程短、功能好等优点,可以作为单纯尺骨干闭合性骨折的首选治疗方法,值得临床应用及推广。
Objective： To evaluate clinical application of isolated ulnar shaft fractures treated by ＂Four Actions and Five Steps＂ method. Methods：By the means of multicenter randomized single blind controlled method, 176 patients were divided into ＂Four Actions and Five Steps＂ group and the operation group from March 1,2010 to June 3,2014. The curative effect was evaluated according to the Anderson evaluation standards. The healing time and the incidence of complications were compared also. Results： The excellent rate of the ＂Four Actions and Five Steps＂ group and the operation group were 97.75% and 96.55% respectively. There was statistical significance（P〉0.05）. The healing time of the ＂Four Actions and Five Steps＂ group was less than that of the operation group. There was no significant difference between the two groups （P〈0.05）. Conclusion： The curative effect of ＂Four Actions and Five Steps＂ method was very significant in treating the i- solated ulnar shaft fractures, which have such advantages as easy manipulation, low cost, short course of treatment, as well as the good function. It deserves to be developed in more hospitals.
