摘要
目的:观察四步推拿手法治疗肩关节周围炎的临床疗效。方法:将120例患者随机分为观察组和对照组各60例。对照组应用醋酸泼尼松注射液1 m L与利多卡因注射液2 m L,痛点封闭,每周1次。观察组采用四步按摩法治疗,1次/d。2组均以治疗3周为1个疗程。结果:观察组痊愈14例,显效30例,有效12例,无效4例,总有效率93.33%;对照组痊愈8例,显效24例,有效20例,无效8例,总有效率86.67%。观察组痊愈率、总有效率均高于对照组,2组比较差异有统计学意义(P<0.05)。肩关节上举、外展、内收、前屈、后伸等活动度2组治疗后均明显改善(P<0.05),观察组改善更明显(P<0.05)。VAS及C-M评分2组均较治疗前显著改善(P<0.05),观察组改善更明显(P<0.05)。结论:四步推拿手法治疗肩周炎疗效优于封闭疗法。
Objective： To observe clinical effects of four-step massage manipulation in treating scapulohumeralperiarthritis. Methods： Altogether 120 patients were randomized into the observation group and the control group, 60 cases each group. The control group were applied to prednisone acetate injection, 1 m L, and lidocaine injection, 2m L,blocked in the pain points, once each week. The observation group were treated with four-step massage manipulation,once each day. Three weeks were one course of the treatment for both groups. Results： There were 14 cases cured, 30 cases markedly effective, 12 cases effective and four cases ineffective, total effective rate of the observation group was93.33%; there were eight cases cured, 24 cases markedly effective, 20 cases effective, eight cases ineffective, total effective rate of the control group was 86.67%. The observation group was higher than the control group in cure rate and total effective rate, the difference had statistical meaning（P〈0.05）. The motility of shoulder joint improved including upwards, abduction, adduction, anteflexion, retroflexion and others in both groups after treating（P〈0.05）, the improvements of the observation group were more obvious（P〈0.05）. VAS and C-M scales of both groups improved more significantly compared with before treating（P〈0.05）, the improvements of the observation group were more notable（P〈0.05）. Conclusion： Four-step massage manipulation is superior to block therapy in treating scapulohumeralperiarthritis.
