无创经皮经椎弓根三维定位导向器在经皮椎体成形术治疗骨质疏松性椎体压缩性骨折中的应用效果 被引量:4

摘要 目的研制一种无创经皮经椎弓根三维定位导向器辅助装置,并将其应用于经皮椎体成形术治疗骨质疏松性椎体压缩性骨折,验证其应用效果。方法选取2014年4月~2015年2月于我院行经皮椎体成形术患者67例,随机分为定位器组(34例)及常规组(33例),定位器组采用无创经皮胸腰椎弓根穿刺三维定位器,常规组采用常规术前定位方法,比较两组手术穿刺时间、穿刺次数及术中X线透视次数。结果两组患者均未出现与穿刺相关的手术并发症,伤口愈合良好。定位器组及常规组术中X线透视次数分别为(3.1±1.7)、(5.2±2.6)次,穿刺时间分别为(6.2±3.4)、(10.6±4.3)min,穿刺次数分别为(2.3±1.1)、(3.4±2.3)次,两组比较,差异有统计学意义(P〈0.05)。结论无创经皮经椎弓根三维定位导向器辅助经皮椎体成形术可有效减少术中X线辐射,缩短手术时间,提高手术效率。 Objective To evaluate the clinical application effect of three-dimensional guiding instrument for osteoporotic vertebral compression fractures(OVCFs),and via percutaneous vertebroplasty(PVP). Methods 67 patients who suffered OVCFs in our hospital from April 2014 to February 2015 were selected,and treated by PVP surgery.They were divided into guiding instrument group(34 cases) and normal group(33 cases),guiding instrument group was treated with a non-invasive percutaneous chest and lumbar puncture of the three dimensional locator,and normal group was used conventional preoperative positioning method.The positioning time,positioning frequency and radiation operation between two groups before and during the operation were compared and statistically analyzed. Results No postoperative wound bleeding,infection or other surgery-related complication was observed between two groups,and the healing of the incisions was good.The X-ray exposing frequency was(3.1±1.7)times in guiding instrument group and(5.2±2.6) times in conventional group.The positioning duration time of guiding instrument group and conventional group was(6.2±3.4) min and(10.6 ±4.3) min respectively,the puncture positioning frequencies of guiding instrument group and conventional group was(2.3±1.1) times and(3.4±2.3)times respectively;the differences were significant between two groups(P0.05). Conclusion Percutaneous lumbar vertebral angioplasty with no invasive percutaneous pedicle can effectively reduce intraoperat ive X-ray radiation,shorten operation time and improve operation efficiency.
