摘要
目的 研究鱼精蛋白对离体人肺动脉收缩功能的影响并探讨其可能的发生机制。方法 3 6例肺部肿瘤患者肺叶切除术后分离下肺叶 ,将肺血管随机分为A组 (n =9)、B组 (n =2 7)。A组不损伤血管内皮。用眼科镊子将B组血管内膜剥除 ,去除血管内皮细胞。A组 (加入鱼精蛋白 ) ;B组分为B1~B5小组。B1组 (加入鱼精蛋白 ,n =6) ,B2 组(肝素及鱼精蛋白 ,n =6) ,B3 组 (吲哚美辛 +鱼精蛋白 ,n =5 ) ,B4组 (吲哚美辛 +肝素 +鱼精蛋白 ,n =5 ) ,B5组 (生理盐水 ,n =5 ) ,各组鱼精蛋白浓度分别依次为 40 ,10 0 ,2 0 0 ,40 0 μg/ml,每隔 15min更换一次浓度。肝素浓度为 8u/ml,吲哚美辛浓度为 1× 10 -5mol/L。结果 鱼精蛋白在低浓度时对内皮完整肺动脉收缩功能无明显影响 ,在浓度为 40 0 μg/ml时 ,血管张力下降 ,与对照组 (B5组 )相比 ,差异具有显著性 ;鱼精蛋白及其与肝素复合物对去内皮肺动脉均有收缩作用 ,B1组与B5组 ,B2 组与B5组比较差异有显著性。这种收缩作用可被吲哚美辛抑制。B3 组与B1组 ,B4组与B2 组比较差异有显著性。结论 鱼精蛋白对离体人肺动脉的影响与血管内皮有关 ,内皮完整时显示出舒张功能 ,去内皮时显示出收缩功能 ;鱼精蛋白与肝素的复合物对离体人肺动脉的收缩作用可被吲哚美辛抑制。
Objective To observe the effects and mechanism of protamine on contractility of isolated human pulmonary artery rings. Methods Third-order pulmonary artery segments collected after lobectomy from thirty-six patients with lung tumor were studied in vitro in organ chambers. They were divided into two groups:group A with intact endothelium rings,group B denuded endothelium rings. Group A ( n =9) received 40~400 μg/ml of protamine. (Final organ bath concentration). Group B was sub divided into five groups (B 1~B 5), group B 1( n =6) received protamine (40~400 μg/ml ), group B 2( n =6) received heparin (8 u/ml . Final organ bath concentration) before protamine. Group B 3( n =5) received indomethacin (1×10 -5 mol/L ) before protamine. Group B 4( n =5) received heparin (8 u/ml ) and indomethacin (1×10 -5 mol/L ) before protamine. Group B 5 ( n =5) received only normal saline. Results Protamine could not induce contraction of pulmonary artery with intact endothelium. It caused relaxation of pulmonary artery with intact endothelium when protamine concentration was higher than 400 mg/ml . Protamin and protamine-heparin complexes induced contraction of pulmonary artery with denuded endothelium. The effect of contraction was inhibited by indomethacin. Conclusion Protamine causes relaxation of pulmonary artery with intact endothelium; Protamin and protamine-hepairn complexes can induce contraction of pulmonary artery with denuded endothelium. The effect of contraction can be inhibited by indomethacin.
