摘要
目的观察目标潮气量无创机械通气(target tidal volume noninvasive mechanical ventilation,TVV)对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)并Ⅱ型呼吸衰竭患者的治疗效果。方法选取急性加重期慢性阻塞性肺疾病(acute exacerbation of chronic obstructive pulmonary disease AECOPD)并Ⅱ型呼吸衰竭患者58例随机分为2组:在常规治疗基础上,一组无创呼吸机采用ST模式为ST组;另一组无创呼吸机采用TVV模式为TVV组。观察2组无创通气治疗前后血气指标变化情况以及临床治疗有效率。结果治疗后2组pH、动脉氧分压水平均较治疗前升高,二氧化碳分压水平均较治疗前下降(P<0.05)。2组治疗有效率及住院时间差异均无统计学意义(P>0.05)。结论 TVV可显著改善AECOPD并发Ⅱ型呼吸衰竭患者的临床症状,疗效肯定,应用价值高。
Objective To investigate the curative effects of target tidal volume noninvasive mechanical ventilation（TVV） in the treatment of chronic obstructive pulmonary disease（COPD） with type Ⅱ respiratory failure. Methods Fifty-eight cases of acute exacerbation of chronic obstructive pulmonary disease（AECOPD） with type Ⅱ respiratory failure were divided into two groups randomly, on the basis of conventional treatment, one group was given treatment of ST noninvasive mechanical ventilation（ST group）;another group was given treatment of target tidal volume noninvasive mechanical ventilation （TVV group）, comparison was made in the improvement of the blood gas and the rate of curative effect of two groups through observing the changes of illness and blood gas before and after treatment.Results The pH and arterial partial pressure of oxygen（PaO2） of two groups after treatment improved more than before treatment, arterial partial pressure of carbon dioxide（PaCO2） of two groups after treatment fell more than before treatment （P〈0.05）.The difference of the curative effect rate and hospitalization days between two groups showed no statistic significance（P〈0.05）.Conclusion TVV can improve significantly the symptoms of AECOPD patients with type Ⅱ respiratory failure, the curativeeffect is affirmative, and the application value is higher.
