甲状腺激素水平与冠状动脉病变严重程度的相关性研究 被引量:1

Correlation between the serum thyroid hormone levels and the severity of coronary artery disease

摘要 目的探究甲状腺激素水平与冠状动脉粥样硬化病变严重程度的关系。方法连续选择504例行冠状动脉造影的患者,所有患者均检测游离甲状腺素(FT4)、游离三碘甲状腺原氨酸(FT3)、促甲状腺激素(TSH)水平,同时收集临床资料、生化检查及造影结果。利用Gensini积分评价冠状动脉病变严重程度,根据Gensini积分四分位点,将所有患者分成4组。结果 4组患者间FT3、FT4差异具有统计学意义(P<0.05或P<0.01)。Spearman相关分析显示,FT3、FT4水平越低,Gensini积分越高(r分别为-0.302,-0.137,P<0.05或P<0.01)。多元线性回归分析显示调整混杂因素后,FT3仍是Gensini积分的独立危险因素。结论 FT3与冠状动脉粥样硬化的严重程度呈负相关,它可以作为冠状动脉病变严重程度的一个独立预测因子。 Objective To investigate the relationship between serum thyroid hormone levels and the severity of coronary artery disease( CAD) in the patients receiving coronary angiography. Methods 504 patients undergoing coronary angiography were enrolled consecutively. The serum levels of thyroid stimulating hormone( TSH),free triiodothyronine( FT3) and free thyroxine( FT4) were detected. Additionally,clinical characteristics and angiographic results were obtained.Patients were divided into four groups according to the Gensini score quartile. Results The FT3,FT4 levels were significantly different among four groups( P 〈 0. 05 or P 〈 0. 01). Spearman correlation analysis indicated that the FT3,FT4 levels had negative correlations with the Gensini scores,which means the more serious degree of coronary atherosclerosis( r =- 0. 302,- 0. 137,P 〈 0. 05 or P 〈 0. 01). Multiple linear regression analysis demonstrated that FT3 level was the independent risk factors for the Gensini score,even after adjusting for other confounding factors. Conclusions FT3 level is negatively correlated with the severity of CAD and could be used as an independent indicator for the severity of CAD.
