摘要
本文就有关苯二氮(艹卓)类的应用情况对1954人作了调研,结果表明:(1)服药率为33%,连续服药一年以上为25.55,(2)应用最多是安定,第二是三唑苯二氮(艹卓),(3)长期服药者容易产生药物不良反应,主要是药物依赖性及记忆力下降,老年人容易发生过度镇静,对呼吸功能不全的病人可引起严重的呼吸抑制,甚至死亡,(4)药物依赖性与年龄无明显相关,与服药的长期性及人格倾向有关。
Nineteen-fiftifour persons were invastigated with relation tothe application of benzodiazepine(BZ) for insomnia. Over 90% wereelderlypeople greater than 60 years of age. Most of them were employed in somesort of brainwork and many were retired. The results showed (1 ) benzo-diazepine were frequently preseribed as sleep medicine in the elderly. Inthis group the incidance of users was 33% (645/1954) and 25.5% (492/1954) belonged to the longterm users (used persistantly more than 1 year)with 10 years on average. (2 ) Adverse Drug Reaction (ADR) occurred inaround 74.6% in total and was especially high in the long-term users.(3 ) The most frequentmanifestation of ADR in long-term users was drugdependance as manifested by withdrawal syndrome with various soma-tic or psychological manifestations and/or a strong desire to obtain thedrug in order to avoid unplesant withdrawal syndrom. The incidance ofdrug dependance was 53% (261/492). (4 ) In 2 elderly persons benzodiazepine produced excessive sedation. In 4 cases with pulmonary insufficiency,one small dose benzodiazepine lead to respiratory depression in 2 and deathin the other 2 cases. 'Slow thought' as measured by MFD was sifnificantlymore in the long-term users than the control group ; this might be indicative of global cognitive impairment. (5 ) Factors affecting drug dependancewere further studied and analysed. Among these, duration of treatment,euviromental factor (often used at night) and personality profile (measuredhy Minnesota Multiphasic Personality Inventory 'MMPI' ) were important. In conclusion, the clear cut presence of ADR, especially of drug de-pendence, suggests that long term use of benzodiazepine can be proble-matic. Judicious short term use, individual tailoring and careful monitor-ing, especially in elderly persons would be much safer.
