胃肠疾病患者口腔与胃黏膜念珠菌基因型相关性分析 被引量:1

Analysis of the genetic evolution of Candidafrom oral and stomach mucosa of patients with gastrointestinal diseases

摘要 目的探讨胃肠疾病患者口腔与胃黏膜寄居念珠菌菌株基因型的相关性。方法对154例因消化不良就诊的患者分别采集口腔及胃黏膜标本、分别用念珠菌显色培养基分离鉴定念珠菌,进行内转录间隔区(internal transcribed spacer region,ITS)基因测序并申请GenBank登录序列号,观察同一患者口腔和胃黏膜念珠菌菌种是否一致。在菌种一致患者中,随机抽取其口腔和胃黏膜分离菌株进行多位点序列分型(multilocus sequencing typing,MLST),并分析口腔及胃黏膜念珠菌基因型进化情况。结果 46例(占29.87%)患者口腔与胃黏膜同时检出念珠菌,其中,37例(占80.43%)患者口、胃分离株均为白色念珠菌:包括胃炎患者25例,溃疡患者11例,胃癌患者1例。随机选取来自24例患者的48个分离株进行白色念珠菌MLST分型,结果显示,20例(占83.33%)患者的双标本基因型一致。结论胃肠疾病患者口腔与胃黏膜念珠菌菌种和基因型一致率较高,且消化性溃疡患者感染率高于胃炎患者。 Objective To compare the genetic evolution of Candidafrom gastric and oral mucosa of patients with gastrointestinal diseases. Methods Human oral swabs and gastric mucosa samples were obtained from 154 patients who presented with dyspeptic symptoms or an ulcer.CHROMagar medium was used to isolate and identify Candidastrains.The internal transcribed spacer(ITS)region of Candidastrains isolated from gastric and oral mucosa of the same patient was sequenced and the GenBank Accession No.was determined.The concordance of Candidaspecies in gastric and oral mucosa was observed in the same patient.Oral swabs and gastric mucosa samples from patients with the same species of Candida albicans were randomly selected for multilocus sequencing typing(MLST)of the species.A UPGMA dendrogram was constructed and the evolutionary relationships among all the C.albicans strains from oral and gastric mucosa were analyzed. Results Candidaspecies were detected at both sites(oral and gastric mucosa)in 46patients(29.87%,46/154).The rate of concordance between Candidaspecies detected at the two sites was 80.43%(37/46).C.albicans was isolated from 37 patients,including 25 with gastritis(25/32,78.13%),11 with an ulcer(11/13,84.62%),and 1with gastric carcinoma(1/1,100%).Samples from 24 patients were randomly selected for MLST.Samples from 20patients(20/24,83.33%)had strains of the same genotype in oral and gastric mucosa.According to the UPGMA dendrogram,the genotypes in Clade 18 were the most common genotype in patients. Conclusion There was a high level of concordance between Candidaspecies isolated from oral and gastric mucosa of patients with gastrointestinal diseases and a high level of concordance between genotypes of the strains.The level of concordance of genotypes was higher in patients with a peptic ulcer than that in patients with gastritis.
