四肢长骨骨折患儿弹性髓内钉微创术后感染临床研究 被引量:12

Clinical study on postoperative infection condition of children with extremities long bone fracture received elastic intramedullary nail minimally invasive surgery

摘要 目的研究四肢长骨骨折患儿行弹性髓内钉微创术后感染情况,为四肢长骨骨折患儿治疗提供参考。方法选取2013年3月-2014年12月医院收治的60例四肢长骨骨折患儿为研究对象,随机分为观察组和对照组,各30例,观察组患儿接受弹性髓内钉微创术,对照组患儿接受切开复位内固定,比较两组患儿的感染情况、血清炎症因子及四肢关节功能、活动度。结果观察组患儿的感染率为3.33%,明显低于对照组的20.00%,两组比较差异有统计学意义(P<0.05);观察组患儿血清TNF-α、IL-6、IL-8水平明显低于对照组(P<0.05);观察组患儿Broberg-Morrey评分、Harris评分、HSS评分以及肘关节伸展和屈曲角度、伸膝最大角度、屈膝最大角度均大于对照组(P<0.05)。结论弹性髓内钉微创术有助于减少感染发生,缓解炎症反应,改善关节功能。 OBJECTIVE To study the postoperative infection condition of children with extremities long bone fracture received elastic intramedullary nail minimally invasive surgery so as to provide references for the treatments.METHODS Totally 60 children with extremities long bone fracture in our hospital from Mar.2013 to Dec.2014 were enrolled and randomly divided into observation group and control group with 30 cases each.The observation group received elastic intramedullary nail minimally invasive surgery,while the control group received open reduction and internal fixation.Then the infection status,serum inflammatory factors and four limbs joint function,range of motion were compared.RESULTS The infection rate of the observation group was 3.33%,which was significently lower than that of the control group with the rate of 20.00%(P〈0.05).The serum TNF-a,IL-6,IL-8levels of the observation group were lower than those of the control group(P〈0.05).Broberg-Morrey score,Harris score,HSS score and elbow extension,flexion angles and maximum extensor angle of knee,flexion angle in the observation group were higher than those of the control group(P〈0.05).CONCLUSIONElastic intramedullary nail minimally invasive surgery is helpful to reduce infections,relieve inflammatory reaction,and improve joint function.
