摘要
目的研究交锁髓内钉配合自主体位治疗模式在股骨骨折中的疗效,以促进断端愈合,改善关节功能。方法选择股骨骨折患者作为研究对象,给予髓内钉内固定治疗,并根据术后体位治疗模式分为采用自主体位治疗的观察组和配合体位治疗的对照组,观察骨折愈合、髋关节和膝关节功能、生活能力和负面情绪。结果观察组影像学愈合54例、愈合时间(125.71±13.41)d、完全负重下地时间(86.93±17.03)d、疼痛程度(NRS)评分(3.15±0.46)分、焦虑情绪(HAMA)评分(18.71±1.96)分、抑郁情绪(HAMD)评分(12.49±1.63)分均明显短于对照组;髋关节功能(Harris)总评分(66.31±7.33)分、膝关节功能(HSS)总评分(92.18±9.63)分、日常生活活动能力(Barthel)评分(72.34±7.93)分均明显高于对照组,术后感染率为1.72%,明显低于对照组的6.45%。结论髓内钉配合自主体位治疗模式能够促进断端愈合、改善关节功能,降低术后感染率,具有积极的临床价值。
OBJECTIVE To study of the effect of interlocking intramedullary nail with independent position mode on treatment of femoral fracture so as to promote the healing of fracture and improve the joint function. METHODS The patients with femoral fracture were collected and given interlocking intramedullary nail for treatment and then were randomly divided into the observation group given independent position mode and the control group given dependent position mode. The postoperative fracture healing, hip and keen function, negative emotions, and living ability were observed. RESULTS The number of the patients with radiographic healing (54 cases), healing time (125.71±13.41)d, full weight-bearing time (86.93±17.03) d, NRS (3.15±0.46), HAMA (18.71±1.96), and HAMD (12.49±1.63) score of the observation group were significantly less than those of the control group. The Barthel score(72. 34±7.93), HSS score(92.18±9.63), and Harris score (66. 31±7.33) were significantly higher in the observation group than in the control group. The incidence of postoperative infections of the observa-tion group was 1.72 %, significantly lower than the control group. CONCLUSION The interlocking intramedullary nail combined with autonomic postural therapy has positive significance in promoting the healing of femoral frac-ture, improving the joint function, and reducing the incidence of postoperative infections.
