摘要
目的初步了解本地区孕妇这一特殊人群乙型肝炎病毒(HBV)感染情况,同时观察HBV感染孕妇不同阳性模式组合丙氨酸转氨酶的变化。方法采用微粒子化学发光法检测2 356例孕妇血清中HBs Ag、HBs Ab、HBe Ag、HBe Ab、HBc Ab 5项乙肝标志物,同时采用酶速率法检测丙氨酸转氨酶,并对结果进行统计分析。结果 2 356例孕妇检出HBs Ag阳性者137例,占5.81%,表面抗体986例,占41.85%,大三阳HBs Ag(+)、HBe Ag(+)、HBc Ab(+)的检出率为2.63%,小三阳HBs Ag(+)、HBe Ab(+)、HBc Ab(+)的检出率为1.91%。不同年龄组的HBs Ag检出率差异无统计学意义(P>0.05)。HBV-DNA阳性率和丙氨酸转氨酶异常增高率,在大三阳孕妇患者的血清中检出率明显高于其他2组。结论本地区孕妇感染HBV比率为4.3%,HBV-DNA阳性率和丙氨酸转氨酶异常增高率在HBe Ag阳性的孕妇中高于HBe Ag阴性的孕妇。
Objective To be acquainted with the status of hepatitis B（ HBV） infection and changes of alanine transferase（ ALT） under the different positive model group in pregnant women with HBV. Methods To detect the 5 markers of hepatitis B in 2 356 cases of pregnant women,HBs Ag,HBs Ab,HBe Ag,HBe Ab,HBc Ab,meanwhile,to adopt enzyme rate method to detect alanine aminotransferase,and all the results were statistically analyzed. Results 137 cases with HBs Ag（ ＋） were detected from 2 356 pregnant women,accounting for 5. 81%,986 cases surface antigen,accounting for 41. 85%; the detection rate of HBs Ag（ ＋）,HBe Ag（ ＋）,HBc Ab（ ＋） accounted for 2. 63%; and the detection rate of HBs Ag（ ＋）,HBe Ab（ ＋）,HBc Ab（ ＋） accounted for 1. 91%; The detection rates of HBs Ag at different age groups had no statistical significance on the differences（ P〉0. 05）. The positive rate of HBV- DNA and the abnormality rate of alanine aminotransferase in the group with HBs Ag（ ＋）,HBe Ag（ ＋）,HBc Ab（ ＋） of pregnant women was significantly higher than that of the other two groups. Conclusion For the local pregnant women in this region,the HBV infection rate is 4. 3%. The positive rate of HBV- DNA and the abnormality rate of alanine aminotransferase in pregnant women with positive HBe Ag is higher than that in pregnant women with negative HBe Ag.
