摘要
目的探讨通过连续性血液净化（continuous blood purification,CPB）治疗重症心力衰竭合并肾衰竭（severe heart failure with renal failure,SHFRF）患者的临床疗效。方法选择2012年6月~2014年4月在我院治疗的重症心力衰竭合并肾衰竭患者50例作为观察组,并对其给予CPB治疗;另外选取此前一年半内在我院治疗的重症心力衰竭合并肾衰竭患者50例作为对照组,该组患者曾给予常规的血液透析治疗。记录和比较观察组患者治疗前和治疗后的血气分析、生化检查、生命特征各指标以及APACHEⅡ评分,并比较观察组和对照组患者治疗后半个月的总体疗效。结果治疗后2 d和7 d,观察组患者的血气分析以及生化检查各指标明显优于治疗前,且其APACHEⅡ评分以及生命特征各指标明显优于治疗前,差异有统计学意义（P〈0.05）;治疗后半个月,观察组和对照组的总有效率分别为90%（45/50）和54%（27/50）,差异有统计学意义（P〈0.05）。观察组治疗的安全性明显好于对照组。结论通过CPB治疗能明显改善重症心力衰竭合并肾衰竭患者的心肾功能,提升患者的生活质量,疗效显著,在临床上值得推广应用。
Objective To observe and explore the clinical effect and safety of continuous blood purification for patients with severe heart failure combine renal failure. Methods All 50 patients with severe heart failure and renal failure（SHFRF） in our hospital since June 2012 to April 2014 were selected as the observation group in this study, and another 50 cases with SHFRF in our hospital within a year and a half before were selected as the control group. The control group was treated with conventional hematodialysis while the observation group was treated with CPB. The blood gas analysis index, biochemical examination index, vital signs index and APACHE Ⅱ score were recorded before and after the treatment, while the total efficiency of the observation group half a month after the treatment were compared with the control group. Results In the observation group, the blood gas analysis, biochemical examination index, the vital signs index and APACHE Ⅱ score after the treatment were also obviously better than these before the treatment, the difference was statistically significant（ P 〈0.05）. The total efficiency of the observation group and the control group were respectively 90%（45/50） and 54%（27/50）, the difference was statistically significant（P 〈0.05）, and the therapeutic safety in the observation group was significantly better than that in the control group. Conclusion CPB has a significant effect that significantly improve the function of heart and kidney and enhance the quality of life for patients with severe heart failure and renal failure, thus it’s worth popularization and application in clinic.
