保留主导管腮腺浅叶切除后腺体ECT测定分析 被引量:20

摘要 目的 保留主导管腮腺浅叶切除术后 ,用ECT测定分析剩余深叶腮腺腺体对核素的摄取、分布情况 ,并以健侧对照 ,确定剩余腺体组织的形态和功能。方法 分别在术后一周和四周 ,对 30例患者静脉注射99TCmO4- 3mci 30min后 ,行颜面部前位及左、右侧位显像。采用Elscint-Apex -spx型SPECT测定 ,分析双侧腮腺腺体摄取核素及分布情况。结果 术后一周剩余腮腺核素摄取基本正常 ,分布欠均 18例 ,核素稀疏 12例 ,核素浓聚 2例 ,放射性计数 0 .98± 0 .15 ;术后 4周剩余腮腺腺体外形 ,形态大致正常 ,核素分布均匀 ,患 /健侧位放射性计数 1.0 2± 0 .15 ;正常对照组腮腺放射性计数 1.0 0± 0 .15。结论 保留主导管腮腺浅叶切除后剩余腺体外形、形态和功能恢复正常 ,ECT评定腺体功能方法简便、灵敏 ,测定结果和动物体视学实验结果吻合。 Objective After the operation of superficial lobe parotidectomy with retaining parotid master duct, it is ascertained of the remaining glands' form and function through the determination and analysis of ECT to the absorbing of nuclides by remaining deep residual glands and their distribution, and the supplement of healthy part's comparison.Methods One and four week after operation, the intravenous injection of 99 TC mO 4-3mci is implemented to 30 patients respectively. After 30 minutes, the effect shows in the front, left and right of the face. It can be made the analysis of the absorbing of nuclides by glands on two sides and their distribution through SPECT determination of Elscint-Apex-spx model. Results After a week of operation, the absorbing of the remaining gland nuclide is basically normal. In all 30 cases, 18 are not well-distributed, 12 are the sparsity of nuclide, 2 are the more assembly of it and the radioactive count is 0.98±0.15; after 4 weeks, the remaining glands' form is basically normal, nuclides are well-distributed, the radioactive count of bad/healthy part is 1.02±0.15.In the comparative group of the health, the radioactive count of gland is 1.00±0.15.Conclusion The result reflects that the remaining glands' form and function has been normal. The method of ECT used in the determination of glands' function is simple and sensitive and the result tallies absolutely with the experiment of the research of animal stereology.
