麻醉诱导前预防性注射小剂量地佐辛对抑制舒芬太尼诱发咳嗽反射的作用 被引量:8

Effect of intravenous dezocine premedication on sufentanil-induced cough

摘要 目的观察全身麻醉诱导前预防性注射小剂量地佐辛在抑制舒芬太尼诱发的咳嗽反射中的作用。方法择期全身麻醉手术患者100例,随机分为地佐辛组和对照组各50例,面罩吸氧5min后,地佐辛组静脉注射地佐辛0.1mg/kg、对照组静脉注射等量生理盐水,10min后2组均于5s内静脉注射舒芬太尼0.3μg/kg,2min后依次静脉注射咪达唑仑、丙泊酚、罗库溴铵进行麻醉诱导。记录2组麻醉诱导前、插管前、插管后即刻、插管后1min、插管后3min收缩压、舒张压及心率,比较舒芬太尼注射后2min内2组咳嗽反射发生情况。结果 2组麻醉诱导前、插管后即刻、插管后1min、插管后3min收缩压、舒张压及心率比较差异无统计学意义(P>0.05);地佐辛组咳嗽反射发生率(2%)明显低于对照组(32%)(P<0.05)。结论全身麻醉诱导前预防性注射小剂量地佐辛,对舒芬太尼诱发的咳嗽反射有明显抑制作用。 Objective To evaluate the effect of premedication with small-dose intravenous dezocine on sufentanil-induced cough during the induction of general anesthesia. Methods A total of 100 patients were randomly divided into dezocine group and control group, with 50 patients in each group. After mask oxygen-inspiration, observation group was intravenously injected 0.1 mg/kg dezocine and control group was injected the same volume of normal saline 10 rain before intravenous injection of 0. 3 μg/kg sufentanil within 5 s. After 2 min, induction of general anesthesia was done by intravenous injection of 0.05 mg/kg midazolam, 1.5 mg/kg propofol and 0.8 mg/kg rocuronium bromide. Systolic blood pressure, diastolic blood pressure and heart rate were recorded before induction anesthesia, before intubation, at intubation, 1 min after intubation and 3 min after intubation. The incidence of cough was compared 2 rain after sufentanil bolus between two groups. Results There were no significant differences in systolic blood pressure, diastolic blood pressure and heart rate at all time points between two groups (P〉0.05). The incidence of cough was significantly lower in dezocine group (2%) than that in control group (32%) (P〈0.05). Conclusion Premedication with small-dose intravenous dezocine can effectively prevent sufentanil-induced cough during the induction of general anesthesia.
