A Study on the Effects of Pulsed Nd:YAG Laser Irradiation in Root Canal on Root Surface Temperature

摘要 目的 :探讨不同剂量的脉冲Nd :YAG激光根管内照射与牙根表面温度变化的关系及影响因素。方法 :10 8颗离体单根管牙处理后分为 3组。A组为同剂量激光根管内照射不同牙位的牙 ;B、C两组分别在 15Hz和 2 0Hz频率下分 7种功率根管内照射 ,B组根管壁涂增效剂后再照射 ,测牙根表面中部的温度 ;C组加测根表面下部的温度。结果 :①同剂量激光根管内照射 ,下切牙较上切牙、尖牙和前磨牙根面温度显著升高 (P <0 0 1)。② 2 0Hz和 15Hz照射组随激光功率增大牙根表面温度升高。③根管壁涂增效剂后各功率激光照射后根面升温显著增加 (P <0 0 1)。④同剂量激光照射 ,根尖部较根管中部温度升高更多。结论 :在一定范围内 ,脉冲Nd :YAG激光根管内照射 ,牙根表面温度变化随功率、频率增大而增加 ; Objective:The purpose of this study was to evaluate the effects of the pulsed Nd:YAG laser irradiation in root canals on temperature changes on the root surface in vitro and optimize the parameters of laser irradiation in root canal treatment.Methods: A total of 108 extracted single_root human teeth were divided into A, B and C groups, after the preparation of root canals and removal of their crowns. Group A was evaluated the surface temperature changes of different teeth irradiated with the same output energy. The teeth in Group B and C were irradiated respectively with the frequencies of 15 Hz and 20 Hz, and 7 different output energy, and the inner root canal surface in Group B was coated with a layer of ink, which worked as the initiator.Results: The tooth surface temperature of the mandibular incisor increased more obviously than that of the maxillary incisor, canine and premolar, during root canal irradiation treatment (P<0 01). Significant difference was shown on the temperature elevation when the teeth were irradiated with 2W or higher output power (P<0 01). Temperature increased significantly higher in the teeth with initiator than that without it (P<0 01). With the same parameters of output energy, the surface temperature demonstrated higher on the apical part of the root canal than that in the middle.Conclusion: There is significantly positive correlation between the temperature elevation on the root surface and the average output power and frequency of the pulsed Nd:YAG laser irradiation in the root canal. The shape of tooth, initiator and different parts of the root may also play important roles in the temperature changes of the tooth surface. This study may help optimize the parameters of clinical laser irradiation treatment.\;
