摘要
目的:探讨胃弥漫性大B细胞淋巴瘤的临床及病理特点,减少误诊误治。方法:对所获军事医学科学院附属医院病理科诊断为胃弥漫性大B细胞淋巴瘤1例病例的临床及病理资料进行回顾性分析。结果:本例胃镜检查发现胃窦见直径约6.0 cm以前壁和大弯侧为主、浸润生长的、周边不规则的溃疡型隆起,中央污秽苔,并见紫蓝色血痂,诊断为胃体窦交界部溃疡型癌。后经我院活检病理及免疫组织化学结果证实为非霍奇金淋巴瘤,类型考虑为弥漫性大B细胞淋巴瘤。结论:胃弥漫性大B细胞淋巴瘤临床及病理类型少见,易误诊为低分化癌,应加强对胃淋巴瘤的临床及病理学特征的认识并仔细分析病情,以避免或减少误诊误治。
Objective： Though investigating the dinical and pathological features of gastric diffuse large B cell lymphoma to reduce misdiagnosis and mistreatment. Methods： Clinical and pathological data was retrospectively analyzed in one case of gastric diffuse large B cell lymphoma, which was diagnosed by Affiliated Hospital of Military Medical Sciences. Results： Xhe endoscope found that this lesion had an invasion surrounding irregular ulcer type uplift of a diameter about 6.0 cm in anterior wall and the greater curvature of gastric antrum, with central filthy fur, and it also had a violet blue blood scab. It was diagnosed as gastric antrum junction ulcerative carcinoma. After that, the biopsy and immunohistochemical results confirmed non-Hodgkin＇s lymphoma as the type, and it was considered as diffuse large B cell lymphoma. Conclusion： Clinical and pathological type of gastric diffuse large B cell lymphoma is rare, which is often misdiagnosed as poorly differentiated carcinoma cancer. We should strengthen the clinical and pathological features of gastric diffuse large B cell lymphoma awareness and careful analysis of the condition, in order to avoid or reduce misdiagnosis and mistreatment.
