摘要
目的探讨克拉霉素（clarithromycin,CLA）与鼻用激素配合鼻腔冲洗对改善复发性鼻窦炎患者临床症状的效果。方法选取2013年6月~2014年6月接诊的96例确诊为复发性鼻窦炎患者,随机分为鼻用激素组和克拉霉素组各48例。对鼻用激素组的48例患者进行鼻腔吸入鼻用激素联合鼻腔冲洗的治疗;对克拉霉素组的48例患者在鼻用激素组的治疗基础上给予克拉霉素治疗。观察两组患者流涕、鼻塞、头痛、嗅觉减退的临床表现,并比较两组患者治疗后的术腔清洁时间和上皮化时间及临床症状的改善情况。结果两组患者在治疗前的流涕、鼻塞、头痛和嗅觉减状况评分相比,差异不具有统计学意义（P〉0.05）,治疗后克拉霉素组流涕、鼻塞、头痛和嗅觉减退状况评分均高于鼻用激素组,差异具有统计学意义（P〈0.05）;治疗后克拉霉素组术后术腔清洁时间和上皮化时间明显优于鼻用激素组,差异具有统计学意义（P〈0.05）;克拉霉素组的总有效率93.75%远高于鼻用激素组的70.83%,差异具有统计学意义（P〈0.05）。结论 CLA与鼻用激素及鼻腔冲洗合用有利于复发性鼻窦炎患者临床症状的改善,均有较好的临床效果,但克拉霉素治疗效果更显著,可在临床优选应用。
Objective To analyze the clinical effect of clarithromycin combine with nasal hormone and nasal wash share for recurrent sinusitis.Methods 96 patients diagnosed as recurrent sinusitis as research object were randomly divided nasal hormone group and clarithromycin group.The nasal hormone group was treated with nasal suction nasal hormone（furoic acid mo betamethasone spray）and nasal wash share.The clarithromycin group was treated with clarithromycin on the basis of nasal hormone group treatment.The runny nose,stuffy nose,headache and hyposmia were observed.The cavity clean time and epithelial change time after treatment and the clinical effect were evaluated.ResultsTwo group patients＇ score including runny nose,stuffy nose,headache and hyposmia before treatment did not have statistically significant（P〉0.05）.After treatment,these score for clarithromycin group were much better than that of nasal hormone group（P〈0.05）.Clarithromycin group patients＇ cavity clean time and epithelial change time was less than nasal hormone group（P〈0.05）after treatment.Totally,clarithromycin group patients＇ clinical effective rate（93.75%）was better than that of nasal hormone group（70.83%）（P〈0.05）.Conclusion Treatment with clarithromycin combine with nasal hormone and nasal wash share for recurrent sinusitis is safe and effective.
