摘要
目的 探讨呼吸内科呼吸道感染性疾病病原菌分布及不同菌株微生物敏感性,为临床合理用药,提高临床疗效提供理论依据.方法 回顾性分析我院2013-01~2014-12收治的入住呼吸内科病房(包括普通病房及ICU)的呼吸道感染性疾病患者320例的临床资料,分析感染病原菌状况、不同菌株微生物敏感性等.结果 呼吸道感染性疾病病原菌分布广泛,以革兰阴性杆菌为主,占58.71%,其中以铜绿假单胞菌最多,占26.94%.普通病房与ICU各类感染病原菌的分布无明显不同.呼吸道感染的金黄色葡萄球菌敏感率最高的几种药物分别为万古霉素(100.0%)、利奈唑胺(98.94%)、复方新诺明(78.72%)、左旋氧氟沙星(78.72%);铜绿假单胞菌为亚胺培南(79.66%)、左旋氧氟沙星(71.19%);大肠埃希菌为复方新诺明(88.89%)、左旋氧氟沙星(83.33%)、哌拉西林他唑巴坦(77.78%)、阿米卡星(72.22%).结论 革兰阴性菌是呼吸内科患者呼吸道感染的主要致病菌,不同类别的菌种对抗感染药物的耐药性存在差异,临床上应随时了解呼吸道感染患者病原菌及微生物敏感性,在治疗中合理选择治疗用药.
Objective To investigate the distribution of the respiratory tract infection pathogenic spectrum and its drug susceptibility test, and to provide references for enhancing curative effect. Methods The distribution of respiratory tract infection and its drug suscep- tibility test of patients in Department of Respiratory Medicine from January 2013 to December 2014 in Shaanxi General Hospital of CAPF were retrospectively analyzed. Results Respiratory tract infection pathogens were widely distributed, mainly Gram negative bacilli (58.71%). Among Gram negative bacilli, pseudomonas aeruginosa was the most, accounting for 26.94%. The main drugs with the high- est sensitivity to Staphylococcus aureus in respiratory tract infection were vancomycin ( 100%), linezolid ( 98. 94%), cotrimoxazole (78.72%) ,levofloxacin(78.72%). The main drugs with the highest sensitivity to Pseudomonas aeruginosa in respiratory tract infection were imipenem(79.66%)and levofloxacin(71.19%). The main drugs with the highest sensitivity to E. coli in respiratory tract infection were cotrimoxazole(88.89% ) ,levofloxacin(83.33% ) ,piperacillin tazobactam(77.78%), amikacin(72.22%). Conclusion The Gram negative bacteria are the main bacteria in respiratory tract infection. The different drug resistances exist in different kinds of bac- teria. Pathogens and antimicrobial susceptibility of bacteria should be understood well in the respiratory tract infection for reasonably se- lecting the drugs.
