摘要
目的研究纳米银敷料与重组人表皮生长因子联合治疗Ⅱ度烧伤的疗效。方法选择2012年8月—2013年11月金华市中心医院接诊的75例Ⅱ度烧伤患者进行研究。随机分为实验组和对照组。其中实验组39例,采用纳米银敷料联合重组人表皮生长因子换药治疗;对照组36例,采用单纯的纳米银敷料换药治疗。在烧伤后7d、15 d、25 d对2组患者进行创面分泌物的细菌培养和鉴定,观察并记录创面的愈合率和愈合的时间。结果 2组患者的一般临床资料状况比较差异无统计学意义(P>0.05)。实验组与对照组的浅Ⅱ度烧伤创面的细菌检出率分别为10.00%和38.89%,二者相比差异有统计学意义(χ2=4.3743,P=0.0365),深Ⅱ度烧伤创面细菌的检出率对比差异无统计学意义(P>0.05)。实验组与对照组的浅Ⅱ度烧伤创面在治疗7d后愈合率分别为50.00%与11.10%,二者相比差异有统计学意义(χ2=6.6311,P=0.0100);实验组的深Ⅱ度烧伤创面在治疗7 d后,15 d后,25 d后的愈合率均高于对照组的深Ⅱ度烧伤患者的愈合率,二者相比差异有统计学意义(P<0.05);浅Ⅱ度创面患者中的实验组的愈合时间短于对照组,二者相比有统计学意义(t=8.4170,P<0.0001);深Ⅱ度创面患者中的实验组的愈合时间短于对照组,二者相比差异有统计学意义(t=3.0600,P=0.0042)。结论纳米银敷料与重组人表皮生长因子联合治疗Ⅱ度烧伤的疗效良好,值得临床推广。
Objective To study the efficacy of combination therapy with recombinant human epidermal growth factor (rhEGF) and nano-silver dressings in the treatment of second-degree bums. Methods A total of 75 patients with Ⅱ de- gree burns in our hospital from September 2012 to November 2013 were enrolled and divided randomly into observation group( n = 39) and control group (n = 36 ). The observation group received the treatment with nano-silver dressing and rhEGF, while the control group received the treatment with nano-silver dressings. The bacterial euhure and identification were done on 7 d, 15 d and 25 d after the treatment by burn wound secretions. The healing rate and healing time of the wound were documented. Results General condition of the two groups of patients had no significant difference (P 〉 0.05 ). The bum wound bacterial detection rates of superficial Ⅱ degree of the observation group and the control group were 10.00% and 38.89 % , respectively, there were significant differences ( X^2 = 4. 3743, P = 0. 0365 ) ; the rate of wound bacteria detected of deep Ⅱ degree had no significant difference ( P 〉 0.05 ). In Superficial Ⅱ degree , the healing rates of the observation group and the control group after 7 days of therapy were 50.00% and 11.10% , there were statistically sig- nificant( X^2 = 6. 6311 ,P = 0. 0100);in deep Ⅱ degree , the healing rates of the observation group and the control group after 7 days, 15 days and 25 days of therapy, both of them had statistically significant difference (P 〈 0.05 ) ;healing time of superficial Ⅱ degree of in the observation group was shorter than healing time of superficial ]] degree of the control group, there were statistically significant difference ( t = 8. 4170, P 〈 0. 0001 ) ; healing time of deep Ⅱ degree bum pa- tients in the observation group is shorter than that in the control group, there were statistically significant difference ( t = 3. 0600, P=0. 0042). Conclusion The efficacy of combination therapy with recombinant human epidermal growth factor and nano-silver dressings for Ⅱ degree burns is well. It is worthy of promotion in clinic.
