摘要
目的:比较0.25%、0.5%、0.75%浓度的利多卡因行星状神经节阻滞的有效性和安全性。方法:对需要进行星状神经节阻滞治疗的150例患者,随机分为Ⅰ、Ⅱ、Ⅲ组,每组各50例,分别以浓度为0.25%、0.5%、0.75%,容量均为10ml的利多卡因,采用第六颈椎(C6)平面气管旁入路法行单侧星状神经节阻滞。观察患者阻滞效果、起效和持续时间、并发症和对重要生命体征的影响。结果:Ⅱ、Ⅲ组有效率比Ⅰ组明显高(P<0.05)。三组间阻滞起效时间比较无统计学意义,Ⅱ、Ⅲ组持续时间比较差异无统计学意义,Ⅰ组持续时间较Ⅱ、Ⅲ组短;150例中出现一过性上肢异常感觉12次(8%),声音嘶哑18例(12%),咽部不适感31例(20.67%),头晕2例(1.3%)。全部病例中无一例局麻药中毒、臂丛神经阻滞、全脊髓麻醉等并发症出现;各组间在星状神经节阻滞(SGB)前后心率、血压、血氧饱和度、呼吸频率比较无统计学意义。结论:0.5%利多卡因的阻滞有效率、起效时间、持续时间与0.75%利多卡因相当,副作用少,安全性高,建议采用0.5%利多卡因。
Objective To compare the concentration of 0.25%,0.5% and 0.75% lidocaine efficacy and safety applyed in the stellate ganglion block (SGB).Methods The need for stellate ganglion block treatment of 150 cases of patients ,random divided intoⅠ,Ⅱ,Ⅲ groups,50 cases in each group,with concentration of 0.25%,0.5% and 0.75% respectively,the capacity of 10 ml of li-docaine,adopting unilateral SGB beside the trachea at C6 plane Horner syndrome were observed and its emergence and the performance of the continuous time,adverse reactions and complications,and the effects on important vital signs.Results Ⅱ,Ⅲ group are higher than the Ⅰ group effective rate (P 〈0.05);Three groups of work time is difference had no statistical significance ;There are no sig-nificant difference in maintain duration time between group Ⅱ、Ⅲ;Ⅰ group duration time shorter than the Ⅱ,Ⅲ group (P &lt;0.05);Three groups feel abnormal transient upper limbs 12 times (8%),hoarseness 18 cases (12%),throat discomfort 31 cases (20.67%),dizziness 2 cases (1.3%).All the cases without a case have a local anesthetics poisoning,brachial plexus block,and the whole spinal anesthesia complications;There was no significant difference in heart rate,boold pressure,SO2 ,respiratory rate be-fore and after SGB.Conclusion 0.5% lidocaine block efficient,work time,duration of 0.75%,lidocaine and less side effects, high security,recommended 0.5% lidocaine.
