产NDM-1型碳青霉烯酶液化沙雷菌耐药性研究 被引量:1

Study on drug resistance of New Delhi metallo- β- lactamase-1( NDM- 1)- producing Serratia liquefaciens

摘要 目的研究产NDM-1型碳青霉烯酶液化沙雷菌的临床和微生物学特点。方法通过VITEK-2型全自动微生物分析仪对临床分离菌株进行鉴定,利用PCR技术来鉴定NDM-1基因,抗菌药物MIC使用肉汤稀释法进行测定。同时对产NDM-1型液化沙雷菌感染的患者的临床资料进行调查分析。结果在本院儿科患者的痰标本中发现1株NDM-1型的液化沙雷菌。药敏结果显示该细菌对β-内酰胺类抗生素耐药,对氯霉素中介,对庆大霉素、左氧氟沙星、阿米卡星、复方新诺明、环丙沙星、米诺环素敏感;进行接合实验显示bla NDM-1能被转移到大肠杆菌J53中。结论产NDM-1型液化沙雷菌细菌的出现,说明bla NDM-1可能已经开始在肠杆菌科细菌中广泛传播,因此临床必须采取严格的措施来控制抗生素的使用,同时,需要采取更加严格和规范的院感控制措施,防止此类细菌的暴发传播。 Objective To research the clinical and rnicrobiological characteristic of NDM- 1- producing Serratia liquefaciens.Methods Use the VITEK- 2 to identify the bacreria,and the MICs of antimicrobial according to broth dilution method. The NDM- 1 gene was identified with PCR. At the same time we collect and analyze the clinical date of patients infected with NDM- 1 carbapenemase- producing Serratia liquefaciens. Results One NDM- 1 type Serratia liquefaciens was foundfrom the department of pediatric. Drug susceptibility results shows it resistance to beta lactam antimicrobial,intermediary to chloramphenicol,sensitive to gentamicin,levofloxacin,amikacin,and paediatric compound sulfamethoxazole tablets,ciprofloxacin and minocycline. Conjugation shows bla NDM- 1 can be transferred to E. coli J53. Conclusions The presence of NDM- 1 in Serratia liquefaciens suggests the possibility of broadly transmission in Enterobacteriaceae bacteria. So the judicious and strict use of antibiotics must be taken,meanwhile more strict and standard of control measures should be taken to prevent it outbreak.
