我院阑尾炎手术患儿病原菌分布及耐药性分析 被引量:3

Pathogenic Bacteria Distribution and Drug Resistance Analysis of Children with Appendicitis Surgery in Our Hospital

摘要 目的:探讨儿童急性阑尾炎的病原菌分布及其药敏情况,为临床合理选用抗菌药物提供依据。方法:收集我院2010年1月至2014年4月阑尾炎手术患儿724例,其中脓液培养患儿348例,分析药敏试验结果。结果:阑尾炎手术脓液细菌培养阳性患儿183例,共检测出病原菌193株。大肠埃希菌占77.72%,其中产超广谱β-内酰胺酶(ESBLs)大肠埃希菌检出率逐年增加。大肠埃希菌对头孢西丁、哌拉西林/他唑巴坦、头孢哌酮/舒巴坦、碳青霉烯类均敏感,而对头孢曲松、头孢唑林、氨苏西林/舒巴坦和氨苄西林耐药率在50%以上。结论:对阑尾炎患儿进行抗感染治疗时,可以经验性选用头霉素类、哌拉西林/他唑巴坦、头孢哌酮/舒巴坦、碳青霉烯类等作为抗感染治疗。 Objective: To analyze the pathogenic bacteria distribution and drug resistance in children with acute appendicitis,and to supply the foundation of the clinical rational use of antibiotics. Methods: From January 2010 to April 2014,totally 724 cases of appendicitis surgery in children were collected. The results of drug resistance of 348 patients with pus culture were retrospectively analyzed. Results: One hundred and eighty three cases of the appendicitis surgery pus culture positive patients were detected,the pathogens were 193 cases,mainly Escherichia coli accounted for 77. 72%. The proportion of positive extended spectrum β-lactamases( ESBLs) increased year by year. Escherichia coli was sensitive to cefoxitin,piperacillin/tazobactam,cefoperazone/sulbactam,carbapenems and almost resistant to ceftriaxone,cefazolin and ampicillin. Conclusion: Escherichia coli is still the major pathogenic bacteria in children with acute appendicitis. We empirically select cephamycins,piperacillin / tazobactam,cefoperazone / sulbactam,carbapenems,etc.
