摘要
目的：探讨应用肌皮瓣联合持续负压封闭引流（Vacuum Sealing Drainage，VSD）技术修复慢性皮肤放射性溃疡的临床效果。方法：自2012年5月至2014年5月郑州大学第一附属医院整形外科采用肌皮瓣联合VSD技术修复慢性皮肤放射性损伤6例，臀大肌肌皮瓣3例，胸大肌肌皮瓣2例，腹直肌肌皮瓣1例。结果：采用肌皮瓣联合VSD术，6例患者移植皮瓣均成活、愈合。术后随访6月～3年，无溃疡复发或形成新的溃疡。结论：肌皮瓣血供丰富，组织量大，充填效果及外观良好，围手术期联合VSD技术清洁创面，促进肉芽生长，同时清除创面内渗液及液化组织，是修复严重放射性溃疡和功能重建行之有效的方法。
Objective Researching the therapeutic method of the chronicity radiation ulcer and the clinical effect of the application of myocutaneous flap combined with vacuum sealing drainage （VSD）. Methods We gather six cases with chronicity radiation ulcer, which repaired by myocutaneous flap combined with vacuum sealing drainage （VSD） from may 2012 to may 2014. There were three cases of gluteal myocutaneous flaps, two cases of pectoralis major myocutaneous flap, and one case of transverse rectus abdominis myocutaneous flaps. Results Six cases of the transplantation of skin flap all get satisfaction results after the therapy of chronicity radiation with the myocutaneous flap combined with VSD, followed up of six months to three years after the operation, no ulcer recurrence or new ulcer appeared. Conclusion The myocutaneous flap have abundance of blood supply and the tissue can get a better filling effect and appearance. It can make the wound surface clearing, promote the granulation growth and remove the secretions at the same time. Overall, it can be an effect ways to repair the severity radiation ulcer.
