摘要
目的探讨百草枯中毒的肺部CT表现与临床影响因素的关系。方法观察48例口服百草枯中毒患者的肺部CT表现并测量肺损害范围。根据患者服药剂量分为:服药〈20ml(小剂量组)、服药20-40ml(中等剂量组)、服药〉40ml(大剂量组)。按中毒时间又可分为:早期组(1周内)、中期组(1-2周)、晚期组(〉2周)。比较分析三组间患者肺部损伤程度与服药剂量、服药后随时间变化情况。结果 48例患者肺部损伤的CT表现主要为:肺纹理增多、胸膜下线、肺过度通气、磨玻璃影、肺实变、肺纤维化、支气管扩张、胸腔积液。百草枯中毒早期肺部损伤CT主要表现为肺纹理增多、磨玻璃影;中、晚期肺部损伤CT主要表现为肺实变、纤维化。肺部损伤程度与服药剂量的差异有统计学意义(P〈0.05)。结论百草枯中毒肺部损伤程度随着口服剂量的增加而加重,百草枯中毒肺损伤的CT表现对判断中毒程度及制定个体化治疗方案和判断愈后具有重要的临床意义。
Objective To discuss the performance of pulmonary CT and the relationship between CT and the clinical factors on paraquat poisoning. Methods Choosing 48 patients who took paracluat orally as objects, their performance of pulmonary CT were observed and the scope of their lung damage were measured. According to the dose, they were divided into three groups: small doses(〈 20ml). moderate doses (20-40ml), large doses (〉 40 ml), and to the time of poisoning: early stage(less than one week), middle stage(one week to two weeks),late stage(more than two weeks). The changes of the three groups on the degree of lung injury between the doses they took and timing after they took were compared. Linear regression analysis were used to analysis the results (P〈0.05 means statistically significant differ- ences). Results The performance of lung damage CT of 48 patients are increased lung markings, pleural subsidence, pulmonary hyperventilation, ground glass, pulmonary consolidation, pulmonary fibrosis, bronchiectasis, pleural effusion. The early stage showed increased lung markings and ground glass. The middle and late stage showed pulmonary consolidation and pulmonary fibrosis. The differences between lung damage level and doses they took orally are statistically significant(P〈0.05). Conclusion The lung damage of paraquat poisoning aggravated level who took orally is related to increasing dose. To know the lung damage CT performance of paraquat poisoning has an important meaning for the clinic to determine the extent of poisoning, develop individualized treatment and evaluate the healing.
