慢性鼻-鼻窦炎筛骨骨质病理改变程度与患者主客观评分及肺功能相关性分析 被引量:4

摘要 目的研究慢性鼻-鼻窦炎筛骨骨质的病理改变程度与患者主客观评分及肺功能相关性分析。方法126例慢性鼻-鼻窦炎患者,男70例,女56例,年龄23~57岁。取患者病变侧的筛窦骨质,行HE染色,并按Biedlingmaier分级方法分析骨质病理改变程度与VAS评分、Lund-Mackay评分、Lund-Kennedy评分、外周血嗜酸性粒细胞计数、肺功能指标的相关性。结果骨质病理改变程度与VAS、Lund-Mackay评分、Lund-Kennedy评分、外周血嗜酸性粒细胞数呈正相关性,与FEV1、FEV1%FEV、PEF、FEV25-75改变成负相关。结论慢性鼻窦炎患者筛骨的病理学改变在一定程度上反映了疾病的严重程度,随着筛骨病理分级的增高,外周血嗜酸性粒细胞计数也相应增高。筛骨病理改变可能与肺功能的改变呈负相关。 Objective To investigate the pathologic characteristics of ethmoid bone with chronic rhinosinusitis( CRS) and to analyze the correlation between pathological grade and subjective/objective scores,pulmonary function in patients with CRS.Methods 126 patients with CRS( 70 males,56 females) were included in this study.They were between 23 and 57 years old.Ethmoid bones were collected during operation.HE staining was used for observing the pathological change and Biedlingmaier criterion was used for determining the pathological grade of ethmoid bone.The correlations between the pathological grade and scores of VAS,Lund-Mackay,and Lund-Kennedy,eosinophil count,indexes of pulmonary function including FEV1,FEV1% FEV,PEF,FEV25-75 were analyzed.Results Pathologic changes of ethmoid bone in CRS were positively correlated with scores of VAS,Lund-Mackay and Lund-Kennedy,eosinophil count.And the changes were negatively correlated with FEV1,FEV1% FEV,PEF and FEV25-75.Conclusion The pathologic changes of ethmoid bone reflect,at some degree,the severity of CRS.Along with the increase of pathologic grade of ethmoid bone,peripheral eosinophil count will increase.The pathologic changes of ethmoid bone maybe negatively correlated with the changes of pulmonary function in patients with CRS.
