胃肠道间质瘤18例临床特点及病理学形态和免疫组化分析 被引量:4

摘要 目的 探讨胃肠间质瘤(gastrointestinal stromal tumor,GIST)的临床病理特征。方法 回顾分析18例GIST的临床特征,病理大体及镜下特点和免疫表型,恶性诊断指标。结果 18例GIST中,男性11例,女性7例,年龄45~75岁。7例发生于胃,2例位于食管,7例位于空肠和十二指肠,2例位于结肠。良性15例,恶性1例,交界性2例。临床表现为腹胀、腹痛、腹部包块、血便等。18例中梭形细胞型13例,上皮细胞型2例,混合细胞型3例。免疫组化:17例CDll7(+),12例DOGl(+),15例CD34(+),vimentin18例(+),2例SMA(+),1例desmin(+),1例S-100(+)。结论 胃肠道间质瘤好发于中老年人,CDll7、DOGl和CD34联合检测可提高GIST的诊断率,手术是重要和有效的治疗方法。
