摘要
目的:对生长发育迟缓儿童进行综合干预研究,旨在创建一套具有现代民政特色的儿童福利机构生长发育迟缓儿童综合干预模式。方法:依据生长发育迟缓儿童的病因及高危因素、临床表现采取整体、多学科参与的综合干预措施。结果:病因及高危因素干预方面:院内75例孤残儿童(具有手术指征)手术前后体格发育水平存在显著性差异(P<0.01),手术干预效果明显;抗癫痫药物干预74例癫痫孤残儿童,治疗有效率78.4%,其病情得到明显缓解;临床表现干预方面:13例营养不良孤残儿童经肠营养、肠外营养干预其体重增加速率与正常儿童比较P<0.01,出现部分赶上生长;150例精神发育迟滞孤残儿童经特殊教育和生活园区送教干预其智力水平都有明显提高;34例运动发育迟缓孤残儿童经群体康复训练干预其粗大运动功能及精细运动功能得到显著改善;16例语言发育迟缓孤残儿童经语言训练其语言理解、语言表达前后比较均有统计学意义(P<0.01),依恋关系和同伴关系的重建为孤残儿童心理健康发展打下良好基础。结论:对于病因复杂,临床表现多方面的生长发育迟缓孤残儿童有必要进行整体、多学科参与的综合干预,发挥他们最大的潜能,使其得到比较全面发展。
Objective:To perform a comprehensive intervention study for growth retardation children, aiming to create a set of comprehensive intervention model with modern civil characteristics for those children in childrenˊs welfare institution. Methods: Ac-cording to the etiology as well as clinical manifestation for the growth retardation children, overall and multidisciplinary comprehensive intervention measures should be adopted. Results:In the etiological intervention, there were significant differences in 75 disabled chil-drenˊs physical development levels (with operative indications) before and after the operation in our institution (P﹤0. 01) and the sur-gical intervention was effective;the treatment efficiency of antiepileptic drug intervention for 74 children with epilepsy was 78. 4%, and their illness was relieved obviously. In the clinical manifestation intervention, the weight growth rate of 13 malnourished disabled chil-dren (intervention of enteral nutrition and parenteral nutrition) was lower than that of those normal children (P﹤0. 01), but a part of children had occurred catch-up growth;during special education and teaching intervention by the nurses in the living garden, the intel-ligence level of 150 mental retardation children had increased significantly; during the community rehabilitation training intervention, the gross motor function and fine motor function of 34 sports retardation children had improved greatly;during linguistic training, there were statistical differences in the language comprehension and language expression for 16 language retardation children (P﹤0. 01). The reconstructions of attachment and peer relationships had laid a good foundation for the development of disabled childrenˊs mental health. Conclusions:For growth retardation children with complex etiology and various clinical manifestations, it is necessary to im-plement a whole and comprehensive intervention of multi-disciplinary participation for them to perform maximum potential and have comprehensive development.
