摘要
目的了解拱墅区围产儿出生缺陷发生情况,探讨出生缺陷发生的相关危险因素,为采取有针对性的干预措施提供依据。方法收集2009年~2013年拱墅区四家产科单位分娩的34 764例围产儿进行出生监测,统计出生缺陷发生率,比较出生缺陷发生情况、构成以及孕期致畸的高危因素。结果共发现出生缺陷儿260例,出生缺陷发生率7.48‰,拱墅区出生缺陷发生率5年间无明显变化;出生缺陷的发生与产妇户籍、出生儿性别、产妇年龄有关;出生缺陷前5位依次为四肢畸形、消化系统畸形、心血管系统畸形、头部器官畸形、泌尿生殖系统畸形;孕期致畸形的高危因素主要为母亲因各种疾病服药、自然流产史、生育过畸形或家族有畸形。结论拱墅区近五年来出生缺陷发生率较为稳定;环境污染和生育年龄延迟可能加大出生缺陷的发生;应加强男性围产儿产前保健;在提倡晚婚晚育的同时,要尽量将生育年龄控制在35岁以前,此外还要加强高龄孕妇的产前筛查和产前诊断力度。
Objective：To explore the incidence and the related risk factors of perinatal birth defects in Gongshu District,provide evidence for the intervention measures. Methods：Monitoring of four maternity units in 34 764 cases of perinatal birth from 2009 to 2013;Statistics and compare the incidence of birth defects statistics,composition and high risk pregnancy teratogenic factors. Results：There were 260 cases of birth defects,the incidence of perinatal birth defects was 7.48 ‰,the incidence of birth defects in 5 years didn′t change significantly;The incidence of birth defects are associated with maternal household registration,the birth sex,maternal age;The top 5 birth defects are deformed limbs,digestive system abnormalities,cardiovascular malformations,head organ malformation,genitourinary system malformations;The primary risk factors for mother because of various diseases,medication history of natural abortion,birth defects or family malformation. Conclusion：In the past five years,the incidence of birth defects is relatively stable in Gongshu District;The pollution of the environment and the reproductive age delay may increase the incidence of birth defects;We should strengthen the perinatal prenatal care;Women should as far as possible birth before age 35;Strengthen the advanced pregnant women prenatal screening and prenatal diagnosis.
