摘要
目的:研究分析慢性萎缩性胃炎的中医证候学特征,以指导临床治疗。方法:将172例慢性萎缩性胃炎患者,根据中医辨证,分为脾胃虚寒、胃络瘀血、肝脾失调、胃阴亏虚4个证型,然后调查其症状、舌脉等证候学资料,并进行统计分析。结果:证候出现频率依次为:脾胃虚寒>胃络瘀血>肝脾失调>胃阴亏虚;出现频率高于5次的症状及舌脉依次为:胃脘隐痛>大便稀或溏>舌质淡>脉弱>胃脘胀满>胃脘灼痛>舌质黯>脉涩>胃脘胀痛>胸胁胀痛>食欲减退>胃脘刺痛>泛酸>恶心呕吐>神疲乏力>食后腹胀>口苦>舌质红>脉细数>嘈杂>呃逆嗳气>四肢倦怠>口干;在腺体萎缩方面,脾胃虚寒证较其它证型更为严重,P<0.01,差异具有统计学意义;在肠上皮化生及异型增生方面,胃络瘀血证较其它证型更为严重,P<0.01,差异具有统计学意义。结论:经过对慢性萎缩性胃炎的证候学特征的统计学分析,总结了该病的病机、证型规律,可以用于指导临床治疗。
Objective：To study the TCM syndromes characteristics of chronic atrophic gastritis and to guide the clinical treatment. Methods ： 172 cases of chronic atrophic gastritis patients, according to TCM syndrome, were divided into spleen deficiency, blood stasis, dysfunction of liver and spleen stomach meridian, stomach yin deficiency syndromes. And then we investigated their symptoms, tongue and pulse syndrome data to make the statistical analysis. Results ： The syndrome frequency was spleen and stomach deficiency cold〉stomach collateral stasis〉gastric dysfunction of liver and spleen yin deficiency.The symptoms and tongue and pulse frequency higher than 5 were epigastric pain〉stool or sloppy〉pale tongue〉epigastric fullness of epigastric burning tongue dark veins astringent〉epigastric pain〉Xiongxie pain〉appetlte epigastrlc pain with nausea and vomiting of pantothenic acid〉lassitude〉bloating after eating the bitter〉red tongue slippery pulse number〉noisy hiccup belch lassitude〉dry mouth. In terms of glandular atrophy, spleen and stomach deficiency cold syndrome was more serious with significance, P〈O.O1. As for intestinal metaplasia and dysplasia, stomach collaterals stasis syndrome was more serious with significance, P〈0.01. Conclusion ： Through statistical characteristic of syndrome of chronic atrophic gastritis analysis, it summarized the pathogenesis and syndrome types of the disease, which can be used to guide the clinical treatment.
