摘要
慢性萎缩性胃炎(CAG)是临床常见病、多发病,具有起病隐匿、病因多端、病理变化复杂、病程长、治疗效果欠佳、迁延难愈等特点。沈舒文教授基于多年临床经验,提出了“三维六纲”的辨治方法,将虚实、寒热、滞逆六纲贯穿于证候病理结构的病因、病性及病势三个维度,执简驭繁,认为脾胃虚弱是CAG病机之根本,气机郁滞是CAG发生、发展之关键,认为“通降胃气,胃降脾自升”为治疗关键,并强调时时顾护胃气,活用益气养阴,补虚调滞;清泻湿热,开化气机;消散痰瘀,促食纳谷等治法,在临床应用中卓有成效。本文介绍沈舒文教授基于“三维六纲”辨治慢性萎缩性胃炎经验,并附医案1则加以佐证,以飧同道。
Chronic atrophic gastritis(CAG)is a common and frequently occurring disease in clinic.It has the characteristics of concealed onset,multiple etiologies,complex pathological changes,long course of disease,poor treatment effect,delay and difficulty in healing.Based on many years of clinical experience,Professor SHEN Shuwen puts forward the syndrome differentiation and treatment method of“Three-dimensional and Six Principles”,which penetrates the six principles of deficiency and excess,cold and heat and stagnation into the three dimensions of etiology,disease nature and disease trend of the pathological structure of syndromes.Professor SHEN Shuwen believes that the weakness of spleen and stomach is the root of CAG pathogenesis,and the depression of Qi mechanism is the key to the occurrence and development of CAG,it is considered that“Descend Stomach Qi,and Spleen Rising”is the key to the treatment,and it is emphasized that we should always care for and protect stomach Qi,make active use of supplementing Qi and nourishing Yin,tonifying deficiency and regulating stagnation,clearing away damp and heat,enlightening activities of Qi,dissipating phlegm and blood stasis,promoting food intake and grain intake,which are effective in clinical application.This paper introduces professor SHEN Shuwen’s experience in the treatment of chronic atrophic gastritis based on the“Three-dimensional and Six Outline”,which is supported by a medical case.
