摘要
目的探讨正畸固定矫治后牙釉质脱矿的临床效果,并对相关影响因素进行分析,为临床牙釉质脱矿的治疗提供更好的依据。方法对嘉兴市妇幼保健院2013年5月—2014年7月收治的116例正畸固定矫治治疗的患者进行回顾性分析,观察患者牙釉质脱矿情况,并对相关因素进行单因素和多因素Logistic回归分析。结果116例患者正畸固定矫治治疗后脱矿者有18例,占15.52%。其中以下颌尖牙牙釉质脱矿、上颌侧切牙、上颌尖牙牙釉质脱矿患病率高;经单因素分析发现,年龄、不良饮食习惯、刷牙方式、牙膏的选择、正畸疗程、甜食的摄入频率等是导致牙釉质脱矿的主要危险因素;去除相关混杂因素,对有统计学意义的单因素进行多因素Logistic回归分析,发现患者的年龄、不良饮食习惯、刷牙次数、正畸疗程是影响牙釉质脱矿的独立危险因素。结论正畸固定矫治后牙釉质脱矿是一个不容忽视的问题,临床医师应对正畸固定矫治患者尽早开展釉质脱矿的防治工作。
Objective To analyze the related influencing factors associated with enamel demineralization after fixed orthodontic appliance therapy,and provide basis for the management of enamel demineralization. Methods One hundred and sixty patients undergoing fixed orthodontic appliance therapy from May 2013 to July 2014 in our hospital were studied. The enamel demineralization was assessed and the related factors were analyzed by univariate or multivariate Logistic regression. Results 18 of 116 cases were with enamel demineralization with a rate of 15. 52%,commonly occurred to mandibular canine,maxillary lateral incisor and maxillary canine. Univariate Logistic regression showed that age,poor dietary habit,tooth brushing style,toothpaste choosing,orthodontic treatment and the frequency of sweets intaking were the major risk factors causing enamel demineralization; among the single factor with statistical significant,multivariate Logistic regression showed that age,poor dietary habit,daily tooth brushing frequency and orthodontic treatment were the independent risk factors for enamel demineralization. Conclusion The enamel demineralization after fixed orthodontic appliance therapy is a serious problem,the prevention and treatment for the enamel demineralization should be early carry out.
